with a full-thickness-skin-graft from the
inguinal region or with skin from the
mastectomy procedure.

Preservation of the clitoris on the base of
the phallus to maintain the ability to have
an orgasm post-operatively. The ability to
have an orgasm will be as good as preoperatively. The clitoral organ is
deepithelisized only and will be located at
the base of the penoid, and covered with
skin of the future scrotum. So during
sexual intercourse the clitoris will be
stimulated.


scrotal prosthesis at a later time.

To be eligible for this sex reassignment surgery
(SRS), the following criteria must be met:


The patients should have had hormonal
treatment for at least 6-8 month
The patients should have two
recommendations for the S.R.S, from two
separate clinical psychologists.
Female to male sex reassignment is done under
general anaesthesia and takes between 7-9 hours
of surgery. We prefer to perform all surgeries in
one stage.


creating a phalloplastic with a neo-urethra

(end-to-side to femoral artery), veins
(end-to-end to perisaphenic veins) and
nerves (end to end to inguinal nerves) and
anastomosis of the neo-urethra to the
prolonged urethra, so that the phallus is
Erectile Prosthesis/Scrotal Prosthesis
A second operative procedure is necessary to put
testicle implants (scrotal prosthesis) and a penile
implant (erectile prosthesis).
This is done after 8-12 months when sensitivity is
present in the phallus.
We use a hydraulic implant from the American
Medical Systems (AMS) with a pump (AMS 700)
which is placed in the neo-scrotum. Furthermore,
a reservoir is placed underneath the lower
abdominal wall and two silicon rods are
introduced into the penoid in order to have a
functional erection. Silicon testicles are also
placed in the neo-scrotum.
Complications
Less than 5%, our patients might have:

Stenosis at the anastomosis urethraneourethra. Easily solved by an urologist
by widening with special devices (reoperations are necessary in less than 2%
of all occurring cases and generally will
be done as an outpatient procedure)

Fistulas at the same anastomosis. They
will close spontaneously within 2-3
months. If the closure does not appear to
be complete, a minimal invasive
procedure with local anaesthesia can
provide for a definitive closure.
Complications which could be fatal or any
serious side effects have not been
observed in the past 300 female to male
procedures. We give a one-year-warranty
concerning all complications that might
occur. This means that we will cover all
costs in our clinic and charge no
additional fees if there is a need for a
correction.
Total Costs
For the phalloplasty with breast removal and
removal of the uterus are € 44 000,00 (Jan. 2005)
This includes:

intensive care and hospital stay

One-year-warranty for the result, no extra
fees for complications resulting from the
first procedure.
Total costs for the second procedure erectile
prosthesis:

€ 16.700,00 (Jan. 2005)
All patients have the right to have such an
operation; however, it is difficult to determine if
one’s health insurance will cover the costs.
Although insurance companies usually assume
the costs, this always depends on the country and
the insurance policies which might limit the cost
coverage.
