Here are some phone numbers you can use to see if you have an
appointment scheduled at another VA. Remember that it can take a few days
to a few weeks to schedule the appointment, and then the appointment may
be days or weeks from now.

MRI Follow up:
We requested an MRI for you. These are done in MARTINEZ.
This appointment will be mailed to you. If you need to reschedule your
MRI, you will need to contact radiology/MRI in Martinez- their number is:

You will also have a TELEPHONE follow up visit scheduled with Dr Singer
to discuss the MRI results. For any problems or questions, please call

Telephone Clinic----Do NOT come to the VA.
Telephone clinic means Dr. Singer will call you on the phone. You may
receive a reminder that sounds like an appt at Martinez, but this is a
TELEPHONE CALL. Make sure your official VA telephone numbers are up
to date. You can let Arlicia know which number you would prefer to be
called. Sometimes Dr Singer calls earlier or later than the exact time on the
sheet. Please let us know if it is not OK to leave a message. For any

Physical Therapy
We have requested a physical therapy consultation for you. They will
contact you with an appointment. It is up to you to change or cancel that

Surgery SchedulingIf you are scheduled for surgery, there is usually an appointment the week
before your surgery for a physical, and another appointment to see the
anesthesia doctor or nurse. Make sure to keep these appointments. If you
miss these appointments you may not be able to get your surgery on the
planned day. Anesthesia telephone number is

Do not eat or drink ANYTHING after midnight the night before your
surgery, unless your Doctor, the Nurse doing your physical, or the
Anesthesia provider tells you to take certain medicines with a sip of water.
Please let us know soon if you are ill, have to cancel or change your surgery
