1. Offer:

2. Locate facility of patient’s choice making sure they are participating with Health
Ins. Carrier
3. Book test
4. Call ins. to see if pre-cert needed, if needed notify facility of pre-cert number
5. Schedule after test appt. for Family Footcare follow up
6. Give script to patient w/ diagnosis code
7. Make a copy of script for patient’s chart
*Bone scans: specialty department Nuclear Medicine Dept closes by 4:30 pm
