RECITALS
1.
This agreement is between Queensland Corrective Services (QCS) and the
(insert name).
2.
QCS agrees to participate in (insert name)Foster Care Program for the
temporary placement and accommodation and care of pups/dogs.
3.
QCS agrees to provide temporary accommodation and care for pups/dogs as
may be placed in its care from time to time by (insert name), as set out in the
operative provisions below.
4.
(insert name) is to provide a copy of its policies and procedures/terms and
conditions to QCS and QCS agrees to comply with same, as far as they relate
to (insert name) and the temporary accommodation of pups/dogs.
5.
QCS is to provide a copy of its policies and procedures/terms and conditions
to (insert name)and (insert name)agrees to comply with same, as far as they
relate to QCS participating in the (insert name)and temporary
accommodation of pups/dogs.
6.
(insert name)is to retain all ownership of the pups/dogs during the temporary
placement with QCS.
OPERATIVE PROVISIONS
1.
HOUSING
1.1
QCS agrees to provide temporary accommodation and care of pups/dogs, at
QCS’ premises described as (insert correction facility’s name and
address) (“the temporary placement”).
QCS agrees the temporary placement will be for a period of (insert length)
weeks/months for each pup/dog, unless otherwise agreed in writing, between
QCS and (insert name).
QCS agrees to provide safe and suitable accommodation for the pups/dogs
during the temporary placement.
QCS agrees the pups/dogs will be at all times, accommodated within a
suitably fenced area and use its best endeavours to ensure the pups/dogs do
not escape.
In the event the pups/dogs escape from QCS’ care, QCS will advise (insert
name) as soon as practicable, that the pup/dog has escaped.
QCS will not be liable for any damage or injury caused by the pups/dogs
whilst not in QCS’ care.
QCS will not be responsible for any costs of or incidental to the locating and
recapturing of the pups/dogs by (insert name).
QCS agrees to provide details of the pups/dogs accommodation and care, to
(insert name)upon request.
QCS will use its best endeavours to ensure the dogs are cared for and
handled by appropriate persons within QCS’ premises.

(insert name)agrees to provide QCS with adequate bedding facilities for the
pups/dogs.
2.
TRANSPORTATION
2.1
If at any time the pups/dogs require transportation to and/or from the premises
at which they are being accommodated, QCS will ensure the provision of
safe and appropriate transportation for the pups/dogs.
When transporting the pups/dogs to (insert name)premises, QCS will do so
during (insert name)regular trading hours.

HEALTH SERVICES
3.1
(insert name)will ensure each pup/dog has been medically examined, has
been wormed, flea treated, vaccinated and micro-chipped (unless under 6
weeks of age) prior to being placed with QCS.
If the pup/dog is under the age of 6 weeks at the time the temporary
placement, (insert name)will attend to the worming, flea treatment,
vaccinations and micro-chipping as soon as practicable after the pup/dog
reaches 6 weeks of age.
QCS agrees to notify (insert name)as soon as practicable of any veterinary
needs and requirements of the pups/dogs whilst they are in the temporary
placement.
(insert name)will be responsible for the organising of and meeting the costs
of and incidental to the pups/dogs obtaining veterinary services during the
temporary placement.
QCS agrees to notify (insert name)as soon as practicable of any health
issues or behavioural changes in the pups/dogs during the temporary
placement.
(insert name)will be responsible for organising and meeting all costs of and
incidental to the pups/dogs behavioural treatment/management needs, such
as training and behavioural management, during the temporary placement.
(insert name)will be responsible for organising and meeting all costs of and
incidental to the pups/dogs grooming and up keeping needs, during the
temporary placement. Whenever possible, such grooming shall occur at the
premises when the pups/dogs are housed, unless (insert name)provides
QCS with not less than 24 hours notice of the pups/dogs need for
transportation to other premises to undergo such grooming and up keeping
requirements.

DIET
4.1
QCS agrees to follow the dietary advice and recommendations provided by
(insert name)with regards to the pups/dogs diet, during the temporary
placement.
(insert name)agrees to provide QCS with all the pups/dogs bowls and food
and ensure the provision of an adequate amount of food and dietary supplies,
as may be required, during the temporary placement.

QCS agrees to give the pups/dogs the food as provided by (insert name),
and ensure the pups/dogs diet remains consistent with the advice and
recommendations of (insert name), during the temporary placement.
QCS will use its best endeavours to ensure the pups/dogs are not fed human
food, without the prior approval by (insert name).
5.
EXERCISE
5.1
Subject to the prior approval by (insert name), QCS agrees not to take the
pups/dogs to any public place or remove the pups/dogs from QCS’ premises,
during the temporary placement.
QCS will ensure the Pups/Dogs receive adequate exercise as instructed by
(insert name).
(insert name)will provide QCS with all toys required for the pups/dogs, during
the temporary placement.

COMPLETION OF CARE/RETURN TO (insert name)
6.1
QCS agrees to the return of pups/dogs to (insert name)at (insert
name)request, upon (insert name)providing QCS no less than 24 hours
notice.
Unless otherwise provided for in clause 6.1 above, QCS will return the
pups/dogs to (insert name)on completion of temporary placement period,
unless otherwise agreed between QCS and (insert name).

GENERAL CARE
7.1
QCS will use its best endeavours to ensure the pups/dogs are not harmed by
any persons or injured whilst the pups/dogs are on its premises, during the
temporary placement.
QCS will use its best endeavours to ensure the pups/dogs are adequately
cared for by appropriate persons and receive the care and assistance
required for the general care, health and wellbeing of the pups/dogs, during
the temporary placement.
QCS shall advise (insert name)as soon as practicable in the event the
pups/dogs escape from QCS’ premises, whilst in QCS’ care.
(insert name)agrees to meet the costs of and organise for registration of the
pups/dogs as may be required by a relevant local government authority.

8.
VARIATION
8.1
Any variation to the operative clauses in this Agreement, must be in writing
and will not have effect until such time as both QCS and (insert name)have
executed such variation.
9.
CONFIDENTIALITY
9.1
Unless authorised by QCS, (insert name)shall ensure that it and its servants
and agents do not reveal to any person any confidential information or other
information of the operations, dealings or affairs of QCS’ participation in the
Foster Care Program.
10.
RELEASE DISCHARGE AND INDEMNITY
10.1
Unless liability is not otherwise assignable, QCS will not be liable for any
damage to property or personal injuries caused by the pups/dogs during the
temporary placement, only in so far as it relates to personal injuries of its
servants and employees covered by Workers Compensation and (insert
name)forever releases discharges and indemnifies QCS of any liability
thereto.
