S - Policy also covers other types of lawsuits such as "property damage", "personal injury" (libel & slander) and Non Owned Automobile liability. General Liability Overview Who is considered an ‘Insured’ under this policy? S Association itself S Members registered in good standing with the OSA, but only while participating in activities sanctioned by the OSA. S Referees, S Coaches, S Managers, S Administrators and volunteer workers, but only for acts within the scope of duties performed on behalf of the OSA. General Liability Coverage What activities are covered under this policy? OSA sanctioned… S Games/ Practices/ Try-outs S Leagues S Tournaments, S Camps and clinics. S Coverage also includes training and development of OSA sanctioned referees General Liability Coverage Non Field Related Activities covered; S Team rentals of gyms, indoor facilities, field permits related to a function of the team or association S Registration booths in malls/gyms, etc. S Dances - sanctioned only General Liability Coverage Activities not covered; S Fundraising Activities – solution to follow! S Non OSA sanctioned activities S Higher risk fundraising activities involving amusement devices, animals, or motor sports S Events involving fireworks S If a club/district owns, or is contractually responsible under a long term lease of a facility (Dome/ Gym, for example) S Offices owned by OSA Districts – Solution to follow! General Liability Coverage Process for obtaining certificates of insurance; S Complete a “Certificate of Insurance Request Form” – obtained from OSA or OSA Website.. S Upon completion, forward the form to your district. S The district will then email the form to HKMB HUB S HKMB HUB will process Certificate requests within 24 hours and email the Certificate back to the district, which, in turn, will email it to the requesting club. Alcohol Liability S No exclusion on policy for defense of liquor related claims. Note: if your event includes the sale/distribution of alcohol you will need prior approval/sanctioning from the OSA in order for the coverage to extend. NOT SANCTIONED – NOT COVERED! Accident Insurance; Overview S Accident Insurance provides protection in the event of a sudden, unforeseen and unexpected event causing bodily injury or loss. S Applies while participating in covered activities sponsored and supervised by the policyholder. S Covered activities means: participating in a sanctioned practice or competition in the sport under the supervision and direction of the Policyholder. S Coverage also applies while traveling as a team immediately to and from such activities. Accident Insurance When are considered ‘covered’ under this program? Participants are covered during sponsored and supervised activities of the OSA…. … such as Games, Practices, Tournaments and other sponsored activities. Accident Insurance When does this coverage respond? S When accidental bodily injury results in a covered loss S Not first payor for Accident Medical expenses S Coverage responds as "excess” S Requires all other collectable insurance (such as parent's health insurance) to respond first before the "excess" Medical Accident policy will pay any benefits. Accident Insurance Medical Services Defined Means the costs for: S drugs, medicines, S medically necessary treatment by S diagnostic tests and x-rays, a physician or dentist, S hospital room and board, S use of an ambulance, S dental work to sound natural teeth, S treatment performed by licensed medical professional (if hospitalization would have otherwise been required), S rental of durable medical equipment like wheel chairs or hospital beds, prosthetic appliances, orthopedic appliances or braces. Accident Insurance What Medical Services are not covered? Excess Accidental Medical Expense does not apply to the following charges and services: S for which the Insured Person has no obligation to pay; S for eyeglasses, contact lenses and other vision or hearing aids; S for any injury for which Worker’s Compensation benefits or occupational injury benefits are payable; S for treatment by a person employed or retained by the Policyholder; S for any injury occurring while fighting, except in self defense; S for treatment required for conditions caused by repetitive motion injuries and not as a result of an Accident, including but not limited to: OsgoodSchlatter’s Disease, bursitis, chondromalacia, shin splints, and tendonitis. Accident Insurance S How do I submit a claim under this policy? S Claim forms with instructions for completing and submitting is located on the HKMB/OSA website (www.hkmb.com/osa). S Claims under this policy can be submitted by completing: Accident Medical Expense Claim Form, Insured’s Statement Attending Physician’s Statement (available on website if not obtained through your Physician) Club Official Statement Accident Insurance ACCIDENT CLAIM – REPORTING PROCEDURE Ensure all items are complete on your claim form. Attach copies of fully itemized medical bills and copies of the Explanation of Benefits from the primary insurance carrier. Have the “Attending Physicians Statement” and “Club Official Statement” completed. Travel Insurance Overview S When traveling out of country, only a fraction of medical expenses resulting from an emergency accident or illness would be covered by OHIP and Chartis Insurance program. Pre-existing conditions are not covered by Chartis Insurance program. S Out-of-Country Travel Insurance is advised for each and every trip. S Please visit the website www.hkmb.com/osa to purchase individual or team coverage while out of Ontario. S At a preferred OSA team rate of $2.68 per day, each traveler will be covered for $5,000,000, 24 hours per day, with no deductibles. Travel Insurance - Benefits S Covers injury or sickness requiring emergency hospital confinement or emergency medical treatment while traveling outside your province or territory of ordinary residence in Canada. S Applicant must be a resident of Canada, and insured and/or eligible for benefits under the Government Health Care Plan of the province or territory in which the applicant resides. Clubcombo Insurance Program Program provides: Office and Directors and Officers Insurance coverage for Leagues, Clubs, Districts (Office Only) that are sanctioned members of the Ontario Soccer Association. S Coverage can only be purchased online – benefits include – credit card payment, wordings sent to purchaser’s e-mail instantly S Minimum annual premiums on stand alone office coverage start at $550 combined office/D&O coverage minimum premium is just $900!
