Objectives

Describe the purpose and goals of the Practice Transformation
Project - a nationwide effort that supports ONC's Health Information
Technology Research Center (HITRC), the current Regional
Extension Centers (RECs) Program, and primary healthcare
providers to become meaningful users of EHRs

Identify the key success phases and factors involved in
implementing EHRs in provider practices

Illustrate the Practice Transformation Roadmap and discuss the
phases, processes, and outcomes that ensure successful EHR
adoption and implementation

Describe the tools, resources, and training materials available to
support REC's and providers realize their goals of practice redesign,
transformation, and meaningful use

Identify and develop "best practices" EHR
implementations to assist 100,000 primary healthcare
providers become meaningful users of EHRs by 2012

Implement and use the most advanced health IT

Facilitate the electronic exchange of this health
information

Synthesize information to identify key success factors in
implementing EHRs

Develop a Roadmap to profile best practices throughout
the EHR implementation process

Design integrated toolkits that assess, plan, and
evaluate EHR implementation efforts

Disseminate EHR implementation tool sets to all

Develop and deliver effective training programs to
support EHR implementation and practice redesign
