Reminder!!!!
Per the Collective Bargaining Agreement,
as of the March 1, 2012 application deadline,
there were 3 new requirements required for the
application/renewal process:
1. Required use of new applications and renewals,
available on the nursing pathways website* for the
application or renewal of the following
designations:
– SN III/HH or H III
– SN IV
– NP III
SN III/HH H III application now includes an
expanded exemplar section for reference
Use of new applications is required as of the March
1, 2012 application deadline
2. Signature of a Mentor is required on all clinical
ladder applications and/or renewal packets,
effective March 1, 2012
3. The local Facility Selection Committee will maintain
and make available an up-to-date list of all
Mentors.
