MRSS
Crisis planning for youth with behavioral/emotional
needs, available 24/7/365

PerformCare is Contracted Systems
Administrator (CSA) for CSOC
CSA functions
• Serve as single point of access for youth to Children’s System of Care
•Anyone may contact PerformCare on behalf of a youth; however,
parent/legal guardian must give consent before services can be initiated
• Authorize services, based on most recent clinical information submitted
• Provide 24-hour/day, 7 days/week availability
• Coordinate access to services for eligible youth
• Help youth obtain any necessary specialized behavioral health services
and/or services for intellectually/developmental disabled
• CSA does not provide direct services

Child Family Team (CFT)
Team of family members, professionals, and community residents
organized by the case management entity to design and oversee
implementation of Individual Service Plan
Individual Service Plan (ISP)
Comprehensive, strength-based plan developed by Child/Family Team
that integrates clinical and social services and supports with family and
community resources to address needs of child and family across life
domains in order to support the child, youth, or young adult to remain in
or return to the community, in school and out of trouble

Intellectual/Developmental Disabilities
Supporting children with intellectual
disabilities/developmental disabilities (ID/DD)
As of January 1, 2013 the Children’s System of Care:
•
Manages all services for children with ID/DD
•
Provides services including family support services (e.g.
respite) and residential services
•
One DCF Case Manager per child (either CMO Care
Manager, DCPP Case Worker, or CSOC DD Consultant)

Eligibility
• For individuals under age 18, eligibility is determined by CSOC
• Application materials for individuals under 18 available on
PerformCare website (www.performcarenj.org)
• For individuals age 18 and older, eligibility is determined by DDD
• Application materials for individuals 18 and older available on DDD
website

• For individuals under age 21, services are provided by CSOC
• For individuals age 21 and older, services are provided by DDD

Substance Abuse Services for Children
and Adolescents
Supporting children with substance abuse
challenges
As of July 1, 2013 the Children’s System of Care:
• Authorizes youth who meet specific criteria to receive
substance use treatment services from any of 15
service providers contracted with the CSOC
• Provides services including outpatient, intensive outpatient,
partial care, short & long term residential treatment, and
sub-acute detoxification

assessment (American Society of Addiction Medicine or ASAM)
