Home Language Survey
SAAH Question and Answer Document – October 22, 2012
 Q
How many home language survey forms are
needed in any student file? Is it true that the oldestdated one should remain and all others be destroyed
even if the oldest one is not from the district the student is
currently attending?

A Only one home language survey should be in a
student’s file. That home language survey should be the
original home language survey for the student (i.e., the
oldest-dated one [or a copy of the oldest-dated one]).
Also, if there is already an existing home language
survey for a student, a new one should not be created.
