• The revised Child Health Record now better meets the needs of
parents and practitioners and reflects the changes to the
Maternal and Child Health service implemented in 2009 through
the revised Key Ages and Stages Framework.
The objectives of the CHR
In consultation with key stakeholders, the following objectives were defined:
•
Paper-based, parent-held record of their child’s health, growth and
development
•
Prompt for parents to attend maternal and child health visits and ask
key health, growth and development questions at those visits
•
A device that facilitates key child health and development education
and promotion
The objectives of the CHR
• A document that complements and aligns with the Service’s clinical
practice, including the revised KAS Framework
•
Method of facilitating communication between parents, health
professionals and other service providers.
These objectives informed the development of the revised CHR.
