In the past, I have had occasional common colds and minor illnesses. However, I have not been
diagnosed with any chronic illnesses or undergone any major surgeries. I have not had any significant
hospitalizations or accidents.
Family History
* Hypertension: My father has hypertension and takes maintenance medication for it.
* Diabetes : My father has Diabetes and takes maintenance medication for it.
Lifestyle
As a student, I try to maintain a healthy lifestyle. While my schedule can be demanding, I make an
effort to engage in physical activities such as walking, jogging, or playing sports whenever possible. I
also strive to consume a balanced diet, including fruits, vegetables, lean proteins, and whole grains. I
do not engage in smoking or recreational drug use.

participating in social activities. I maintain healthy relationships with my classmates and find support
within my social circle. I have not experienced any significant stressors or major life changes recently.
