As a nurse in community and in hospital setting how will you able to assess a client with
high-risk pregnancy state. What are nursing interventions that you could give to a client
with high risk in general. (Minimum of 150 words)
Pregnancies at high risk are ones in which the mother's and the fetus's health are put in danger
due to coexisting illnesses, pregnancy-related difficulties, or outside influences. In order to
respond quickly to these emergencies, nurses must be knowledgeable about this condition. As
a nurse, it is my duty to ensure the care of both the mother and fetus is important for a safe
delivery and as a nurse in community and in hospital setting, I will be able to assess a client
with high-risk pregnancy state by considering the needs of the individual. I will start by asking
them about their current health and any medications they are currently taking, and
considering their age, weight, height, and I will also look at the client's past medical history and
review any previous interactions with healthcare providers. I will also look at their lifestyle
habits such as smoking or drinking alcohol which can affect fetal development or cause
complications like preeclampsia or gestational diabetes mellitus (GDM). The first nursing
intervention that I would recommend for a client with high risk in pregnancy is to make sure
that they are eating well and getting enough rest. This is because high-risk women often
experience nausea, which can make it difficult for them to eat or sleep. I will also closely
monitor the client weight gain over time, as well as her blood pressure, heart rate, and
temperature, as well as checking for signs of infection or bleeding.
