PROJECT DESIGN AND MATRIX OF ACTIVITIES
COMMUNITY SERVICE AT PUROK 6-A, MATINA PANGI DAVAO CITY WITH A THEME:
“SPONSOR-A-CHILD, A FACE-TO-FACE GIFT GIVING PROGRAM”
I.
INTRODUCTION:
Community service is unpaid work performed by a person or group of people for the benefit and betterment of their community
without any form of compensation. Community service can be distinct from volunteering, since it is not always performed on a
voluntary basis and may be compulsory.

Community service involvement is important because volunteering teaches people of all ages and backgrounds compassion
and understanding.
One thing I like about community service is that there are opportunities to improve and leave your mark on your global and local
community. Volunteering and putting on service events can be used as a way to advocate for causes that you are personally
passionate about.

II.
INTENT
The intent of this community service is to help those elementary learners at Purok 6-A by giving school supplies like paper,
notebooks, crayons, coloring paper, bond paper and etc. ,and even Personal Protective Equipment’s such as face mask,
alcohol or the like for the protection of the family.
Teachers also will be assigned to elected pupils/students of the community in order to help them in their needs for school
supplies.
III. SCOPE OF PARTNERSHIP
This partnership is intended to establish and participate in a Community Service with participation to a civic society program having
a theme “Sponsor-A-Child, A Face-to-Face Gift Giving Program”.

DATE AND TERM
It has a two year timeline. Please see enclosed Project Design and Matrix of Activities for perusal and reference. Provision in
the Agreement may be modified, altered, or supplemented subject to the mutual agreement of both parties

Work cooperatively to ensure appropriate, efficient communication in support for purpose of this agreement.
Regularly share information about activities’ successes and barriers to success with the intent to improve program/services.
Affix signature as one of the signatories in the “certificate of recognition/appreciation/ participation” to be issued / awarded to
all members- participant/ participating individual.
VI. THE VOLUNTEER TEACHER RESPONSIBILITIES

Gather and implement the delivery School Supplies Kit and Personal Protective Equipment (PPE) to the targeted learnerbeneficiaries.
Inform its members- teacher participants/participating individual to ensure their cooperation, involvement and participation.
Provide the certificate of recognition/appreciation/participation necessary for the members- teacher participants/participating
individual during the conduct of the program/activity.
Provide School supplies kits and PPEs through its members- teacher participants/participating individual.
Provide information to each teacher-volunteer regarding the schedule of the conduct of the program/activity.
Provide and assigned tutorial assistance to the children of the beneficiaries from the teacher- volunteer.
Initiate other services and/or civic actions like clean-up drive, provision of food and medical assistance that will sustain the
social well-being of the families-beneficiaries to better improve their quality of life.
VII. THE OFFICE OR THE BARANGAY CHAIRMAN RESPONSIBILITIES

Convene the families-beneficiaries in attending the conduct of community service in the subject area.
Assign Barangay Police in the imposition of the minimum health protocol set forth by the Inter-Agency Task Force (IATF).
Ensure the attendance of the Purok leader to assist in the execution of the community service.
If needed, provide facilities like tables and chairs during the conduct of community service.
