LICE
As the schools are reopened
after the covid 19 lockdown,
children are very happy to go
physical classes.
So we are facing head lice
as an important problem among
students, even males are prone to
get lice because of the closeness
with female children because it is
highly contagious.
They are spreading through
close head to head contact, sleeping
together, sitting closely when
travelling, bedor bed sheet and
towel sharing, using others hat,
playing closelycomb, helmet
and headphone sharing, hairclip
shsring are the important factors
of spreading infection.
Nits are the eggs stuck on the
hair, they wont spread by close
contact.
Lice lay nits takes a week for
hatching, further week for become
a lice. Again the cycle starts each
lice lay 8 eggs per day. Life cycle
ends by a month itself.

MISTRUST
Born into a family of an
agriculturist, Ramesh had made it to
a degree college. After passing out
as a commerce graduate, he joined
a private bank as a clerk. Within a
period of six years, he had passed
the promotional examinations and
become an officer in the bank.
Fortunately for his family he was
transferred to a branch in his home
town. Being a loyal son, he saw
to it that his two younger sisters
finished their education and got
married. Then only he thought of
getting married. By that time he
was well past thirty years of age
and completed ten years of service
in the bank. His father found him a
suitable bride and got him married.
His wife Kamala had been working
in a Government High School as a
teacher in the neighbouring village.
Being an ideal couple, they enjoyed
their life. Recently Kamala, had
given a good news that she had
become pregnant.
The lonely nights in a remote
village where he had to join
his service in the early years of
his service and the long wait for
his marriage had made him get
addicted to smoking. He never
heeded his father’s advise to quit
smoking. His wife too could not
do anything about that.

On an evening at around 5.30
p.m. after returning from work,
he experienced severe heart burn
and upper abdominal pain. He
attributed this to indigestion since
he had a sumptuous lunch with
horse gram soop and rice biryani.
In addition, there was a send off
party to one of his colleagues at his
bank. He had relished the sweets,
and hot and spicy onion pakodas
washed down with payasam of
poppy seeds with cashew nuts
flavoured with elaichi powder.
Whenever he had a hearty meal,
he used to smoke and he thought
that it would prevent indigestion.
On that day also he smoked many
cigarettes. But the symptoms did
not show any signs of abating.
Hence he asked his wife Kamala to
prepare a household remedy with
jeera, ginger and salt. With a glass
full of that he gulped down two
Dhanvanthari pills, an Ayurvedic
Medicine which he thought would
help him. Still, the pain and burning
sensation in the upper abdomen did
not come down. Hence he asked his
wife to fetch a few antacid tablets
from a nearby drug store. But the
adamant symptoms did not recede.
Now his wife became concerned.
She asked him to go to a hospital
and consult a doctor. “Hey, why are
you panicking? It is a small thing
...... this indigestion and I had just

taken antacid tablets. Let us wait
for a while; I am sure it would
come down with passing of gas.”
So speaking, he went to the toilet
and came back. But the pain did
not subside.
so. To confirm and treat that some
more tests are needed which cannot
be done here. So, I am sending you
to the district hospital, where all the
facilities are available. You have to
go now, the earlier the better.”
By that time his father came
to know of this. He scolded him
for his negligence and asked him
to go to their family doctor at a
nearby hospital. Ramesh had to
oblige and he wanted to go there
on his two wheeler. His father did
not agree for that and he called a
taxie. His wife accompanied him
to the hospital.
By that time his wife Kamala
and gone outside to call his father
and inform. She also called her
father and told him to come to the
district hospital.
By the time he reached the
hospital it was well past 10 p.m.
The physician on duty there
examined him and took an ECG.
(Electrocardiogram) Upon going
through it the physician did not
waste any time. He called his wife
Kamala out of ear shot from him,
and told, “He has an heart attack.
You have to shift him to the district
hospital immediately. They will
do an angiogram and if necessary
will put a stent. I will put him on
necessary drugs now.”
Without a word she agreed to
the doctor’s suggestion. Then he
went near the patient lying on the
examination couch and told, “It
seems you have some problem in
your heart. The ECG readings say

But Ramesh was not ready
to accept what the doctor had
just told him. He shouted at the
doctor, “What doctor, what are
you talking about? I don’t have
any heart problems. I have some
indigestion. I had taken spicy soup
during lunch. The meal was heavy.
In addition, I had to participate in a
fairwell programme of a colleague
in the Bank. I had eaten heartily
there. Upon his transfer, I am being
promoted. I agree, I exceeded my
limit, that is why I am having this
problem. What you have told is
ridiculous. I am 34 years old. I
never had BP, (Elevated blood
pressure) no sugar. I am perfectly
healthy except this heart burn.
Just check me one more time and
see....”. He went on talking like
that. However, the doctor who had
seen many such unbeleiving, non
co-operative patients calmly and
slowly repeated what he had told
earlier, trying to make him accept

the reality and coaxing him to go to
the higher care centre immediately.
He directed the sister on duty to
give him necessary treatment and
started writing a reference letter
to the cardiologist in the district
hospital.
However, the patient refused
to take any treatment, not even
the nasal oxygen. He frantically
searched for his mobile. Since he
could not find it nearby, he thought
that his wife had taken the mobile.
So he went out in search of her,
shouting her name, not heeding
to the words of the hospital staff.
Hearing him shouting, his wife got
alarmed and cut short the mobile
conversation with her father and
hurried towards him. He grabbed
the mobile from her hand and
called his closest friend. As soon
as the friend received the call,
Ramesh started shouting into it.
“Maharaya, where are you? Come
to the hospital immediately. I am
here being sick, I have some gas
trouble due to indigestion for which
I have come here to the village
hospital. The doctor here could not
find out what is wrong with me. He
is thinking that I have some heart
problem and asking me to go to the
district hospital. I think he is after
the cuts out of the bill there. Please
come immediately and shift me to
any other hospital where the doctor
is honest.......” The doctor heard

him shouting into the mobile but
did not lose his calm and asked the
staff to restrian him but to no avail.
His wife told him, “No, no, I have
already engaged an ambulance, we
will start immediately!!
“Hey, what are you talking
about? Are you mad? The doctor
here is no good. You know very
well that, I had taken the hot and
spicy soup you have prepared
which has caused all the problems.
My friend will come now. We will
go to some other hospital in the
taluk to get good treatment..........
Settle the bill here he is after
money...........”
He stopped talking mid way and
staggered and fell to the ground.
The hospital staff rushed to him
and lifted him on to the bed.
The physician, ran towards him,
found that his heart beats were
fast and weak. Hence he started
cardio pulmonary resuscitation.
The trained staff nurse brought
the defibrillator which was two
beds away. The physician gave
a jolt to the patient’s chest with
the pads and patient gasped and
his heart started beating again.
Meanwhile he intubated the patient
connecting it to an Ambubag. The
heart stopped once again and the
doctor gave one more shock but
the heart did not respond.

He had to declare him dead after
about 20 minutes.
Evidently the patient had a
repeated heart attack and developed
arrhythmia and cardiac arrest.
The generalised mistrust
towards the doctors prevailing in
the educated people due to the
recent developments in the medico
- social circumstances compounded
by the arrogance, over confidence
and the young age of the patient
had cost him his life. If he had
accepted the doctor’s suggestion
and opted to go to the higher centre,
there was a better chance of him
getting into the cardiology ICU
- alive. No doubt his excitement,
anger and the resultant actions
have together caused a second heart
attack and cardiac arrest. It was a
tragedy....... he could not see his yet
to be born offspring.
Almost every doctor has to see
in a year one or two brought dead
patients with a strip of antacid
tablets in their pocket. Such
scenario continue to occur. Change
in the attitude of the people, the
media and the government towards
the doctors may bring about a
decrease in such incidents.


STONE MAN SYNDROME
(FIBRODYSPLASIA OSSIFICANS
PROGRESSIVA)
Fibrodysplasia
Ossificans Progressiva
(FOP) is a rare
genetic disorder that
causes soft tissues to
transform permanently
into bone. These bones
grow abnormally in
the muscles, tendons,
ligaments and other connective
tissues, forming bridges of extra
bone across the joints. As a result,
movement in the areas affected
by FOP is greatly restricted
and sometimes impossible. The
condition affects many areas of the
body including, but not limited to,
the neck, spine, chest, shoulders,
elbows, wrists, hips, knees, ankles
and jaw.
Globally, FOP affects only
about 2,500 people, or one in 2
million, of all ethnicities, ages and
races. Because FOP is such a rare
disease, experts believe that 80
percent or more of the cases are
misdiagnosed and its prevalence
may be much higher than known.
Causes:
• Gene mutation responsible for
the condition specially mutation

in a bone morphogenetic
protein (BMP) receptor gene on
chromosome-2 called Activin
Receptor Type IA (ACVR1/
ALK2).
Symptoms:
The primary symptoms of
FOP depend on which parts of the
body have become ossified. The
following symptoms are:
 Trouble eating that may lead
to nutritional deficiencies or
malnutrition
 Difficulty in speaking
 Dental problems
 Difficulty in breathing
 Respiratory infections
 Hearing impairment

symptoms or improve the lifestyle:
• Lab tests to measure alkaline
phosphatase levels
 Avoiding situations that could
result in injury, like playing
sports.
• Genetic testing to look for
mutations.
Treatment:
There is currently no cure for
FOP. There is also no definitive or

 Avoiding invasive medical
procedures such as biopsies,
dental work, and intramuscular
immunizations.
 Use prophylactic antibiotics

walkers or wheelchairs.
 Infection prevention measures
such as proper hand hygiene
to guard against common viral

Introduction
A kidney stone is
a hard object that is
made from chemicals
in the urine. Urine
has various wastes
dissolved in it. When
there is too much
waste in too little
liquid, crystals begin
to form. The crystals
attract other elements
and join together to
form a solid that will get larger
unless it is passed out of the body
with the urine. Usually, these
chemicals are eliminated in the
urine by the body’s master chemist,
the kidney.
There are four types of kidney
stones: calcium oxalate, uric acid,
struvite, and cysteine.
After it is formed, the stone
may stay in the kidney or travel
down the urinary tract into the
ureter. Sometimes, tiny stones
move out of the body in the urine
without causing too much pain. But
stones that don’t move may cause
a back-up of urine in the kidney,
ureter, the bladder, or the urethra.
This is what causes the pain.

Causes
Renal stones are formed when
there is a decrease in urine volume
and an excess of mineral deposits
in the urine such as dehydration.
Formation of stones at any location
in the urinary tract is referred to as
urolithiasis, and it can also be due
to complications of urinary tract
infection (UTI).
Risk factors
1. Lack of water
2. Obesity
3. a family or personal history of
kidney stones
4. sex, as they are more common
in males than females

Symptoms
1. Severe, sharp pain in the side
and back, below the ribs

Cancer is a disease in which
cells in the body grow out of
control. Cancer is always named
for the part of the body where it
starts, even if it spreads to other
body parts later. When cancer starts
in the cervix, it is called cervical
cancer. The cervix connects the
vagina (birth canal) to the upper
part of the uterus. The uterus (or
womb) is where a baby grows
when a woman is pregnant.
All women are at risk for
cervical cancer. Long-lasting
infection with certain types of
human papillomavirus (HPV) is the
main cause of cervical cancer. HPV
is a common virus that is passed
from one person to another during
sex. At least half of sexually active
people will have HPV at some
point in their lives, but few women
will get cervical cancer.

THE IMPORTANCE OF PROPER BODY
MECHANICS - KEEPING YOUR
SPINE HEALTHY
Body mechanics is a term used
to describe the ways we move as
we go about our daily lives. It
includes how we hold our bodies
when we sit, stand, lift, carry, bend,
and sleep. Poor body mechanics are
often the cause of back problems.
When we don’t move correctly
and safely, the spine is subjected
to abnormal stresses that over time
can lead to degeneration of spinal
structures like discs and joints,
injury, and unnecessary wear and
tear.
That is why it is so important
to learn the problems of proper
body mechanics. But don’t worry,
it’s not complicated. And once you
get used to them, they can easily be
incorporated into your daily life.
You will be glad you learned them
as they can save you from back
pain and discomfort.
Posture
We have all been told since
childhood to “stand up straight”.
But it’s easy to get into bad habits.
Good body mechanics are based on
good posture. Good posture means
the spine is in a “neutral” position
- not too rounded forward and not

arched back too far. But what does
good posture look like?
It’s easy, follow these few steps:
• Stand with your feet apart.
• Create a small hallow in your
lower back by tucking the
tailbone in and tilting your
pelvic bone slightly forward.
This is done by tightening the
muscles of the buttocks and
thus, rotating the pelvis into the
neutral position. Be careful not
to arch too much.
• Pull the shoulders back and lift
your chest.
• Lift your chin until it is level
and relax your jaw and mouth.
That’s it, proper posture! Feel
how balanced the spine is? Very
little energy is now required to keep
it that way. Practice this position
until it becomes second nature.
Being aware of your posture
during all of your daily activities
is the best way to ensure you are
using good body mechanics. Here
are a few exercises that can help
improve your posture.

being used. Use the following
guidelines to minimize the risk of
injury to your back when doing
standing work:

• Avoid standing in one position
for prolonged periods of time.
Change your position as often
as you can. This will not only
help relieve stress on your
spine, it also helps increase
circulation and decrease muscle
fatigue. When you can, stretch.
Gentle stretching exercises
during a break can help ease
muscle tightness.
• Be aware of your posture. Are
you standing correctly? Check
and double check throughout
your day.
• Make sure the surface you are
standing on is firm and level.
• If possible, lean on a solid
support. This can help reduce
fatigue during long periods of
standing.
Other important tips to remember:
• When lifting an object, avoid

bending and twisting at the
same time. Instead, face the
object and bend at the knees
(not the back).
• Before lifting, move close to
and directly in front of the
object. Make sure your feet are
flat on the floor and shoulders
are apart. Bend the knees and
lift smoothly. Use the same
movement when putting the
object down again.
• Avoid overreaching. If you
have to reach up to a high level,
make sure you are standing
on a firm level surface. Avoid
standing on tiptoes. Not only
is this an unstable position to
stand in, for prolonged periods
of time it can place unnecessary
strain on the back and neck.

Whether sitting at a desk or
at home watching

to keep in mind.
For deskwork,
consider investing
in an ergonomically

What does proper sitting look like?
• Place your buttocks at the back
of the seat while maintaining a

of your knees and the seat of
the chair.
• Place your feet flat on the floor
with your knees bent at a 90°
angle.
• Pull your shoulders back and
lift your chest.
• Lift your chin until it is level
and relax your jaw and mouth.
If your chair has armrests,
make sure they are positioned to
support the weight of your arms.
Not too high to make you hunch
or too low to make you reach.
Footrests can also be a helpful way
to maintain good posture while
sitting. Make sure the footrest is
positioned so that your knees are
bent comfortably and are level with
your hips.
For prolonged periods of
sitting, make sure you have enough
support for your lower back. Look
for a chair that has adjustable
lumbar support. If that is not
possible, you can increase your

spine even while we are in bed.
Here’s how:
• Make sure you are sleeping on
a firm mattress.

or even a rolled up towel or cushion
placed behind your lower back.
Keep in mind that even sitting
in the “correct” position for long
periods of time will eventually

forget to take breaks, get up, move
around and stretch! This will
reduce the stress on your spine
and help prevent muscle fatigue
and stiffness.
Sleeping
We spend about one-third of our
time in bed, so we can’t ignore how
our bodies are positioned during
sleep. As during our waking hours,
the goal is to maintain a neutral

or with your head elevated
on an oversized pillow. These
positions cause the back to arch
and places stress on the spine.
• The side and back are the best
positions for maintaining a
neutral position and a must
for anyone with back or neck
problems.
• Place a pillow between your
knees (for side sleeping) or
behind your knees (for back
sleeping). This will help keep
your spine in the right position
and help ease stress on the
lower back.
• Use a pillow that allows you to
keep your head aligned
with the rest of your
body. Numerous and/
or oversized pillows
may look great on a
made bed but do not
necessarily benefit your
back while sleeping.
Lifting
The process of lifting places
perhaps the greatest loads on the

highest risk of injury. Use of proper
lifting mechanics and posture is
critical to prevent injury. In the
end, it is more important how you
lift than how heavy a weight you
lift. Here are a few tips
on how to lift safely:

55. Diet in Chronic Kidney Disease
Part 2
In patients suffering from
chronic kidney disease (CKD),
regulation of fluids and electrolytes
may be deranged. To reduce the
burden on the kidney with impaired
function and to avoid disturbances
in fluid and electrolyte balance,
patients with chronic kidney
disease should modify their diet
as per the guidance of the doctor
and the dietitian. Today we will
understand two important aspects
of diet, restriction of protein and
salt in patients with CKD.
Restrict Protein Intake
Protein is essential for the repair
and maintenance of body tissues. It
also helps in the healing of wounds
and fighting against infection.
Protein restriction (< 0.8 gm/kg
body weight/day) is recommended
for CKD patients not on dialysis to
reduce the rate of decline in kidney
function and delay the need for
dialysis and kidney transplantation.
Severe protein restriction should be
avoided however because of the
risk of malnutrition. Poor appetite
is common in CKD patients. Poor
appetite and strict protein restriction
together can lead to poor nutrition,
weight loss, lack of energy, and

reduction in body resistance, which
increase the risk of death. Proteins
with high biologic value such as
animal protein (meat, poultry, and
fish), eggs, and tofu are preferred.
High-protein diets (e.g. Atkins
diet) should be avoided in CKD
patients. Likewise, the use of
protein supplements and drugs
such as creatine used for muscle
development is best avoided
unless approved by a physician or
dietitian. However, once a patient
is on dialysis, protein intake should
be increased to 1.0 – 1.2 gm/kg
body weight/day to replace the
proteins lost during the procedure.
Salt (Sodium) Restriction in Diet
Why is a low sodium diet advised
for patients with CKD?
Sodium in our diet is important
for the body to maintain blood
volume and to control blood
pressure. Our kidneys play an
important role in the regulation

of sodium. In patients with CKD,
the kidneys cannot remove excess
sodium and fluid from the body
so sodium and water build up in
the body. An increased amount
of sodium in the body leads to
increased thirst, swelling, shortness
of breath, and an increase in blood
pressure. To prevent or reduce
these problems, patients with CKD
must restrict sodium intake in their
diet.

cakes, pizza, and bread
How much salt should one take?
9. Vegetables like cabbage,
cauliflower, spinach, radish,
beetroot, and coriander leaves
A typical daily intake of salt is
about 10 to 15 grams (4-6 grams
of sodium) per day. Patients with
CKD should take salt according
to the recommendation of the
doctor. CKD patients with edema
(swelling) and high blood pressure
are usually advised to take less than
2 grams of sodium per day.
Which foods contain high amounts
of sodium? Foods high in sodium
include:
1. Table salt (common salt),
baking powder
2. Processed foods like canned
foods, fast foods, and “deli”
meats
3. Readymade sauces
4. Seasonings and condiments
such as fish sauce and soy sauce

6. Wafers, chips, popcorn, salted
groundnuts, salted dry fruits
like cashew nuts and pistachios
7. Commercial salted butter and
cheese
8. Instant foods like noodles,
spaghetti, macaroni, and
cornflakes
10. Coconut water
11. Drugs like sodium bicarbonate
tablets, antacids, laxatives
12. Non-vegetarian foods like meat,
chicken, and animal innards like
kidneys, liver, and brain
13. Seafood like crab, lobster,
oyster, shrimp, oily fish, and
dried fish
Practical Tips to Reduce Sodium
in Food
1. Restrict salt intake and avoid
extra salt and baking soda in
the diet. Cook food without salt
and add permitted amounts of
salt separately. This is the best
option to reduce salt intake
and ensure consumption of the

prescribed amount of salt in
everyday diet.
2. Avoid foods with high sodium
content (as listed above).
3. Do not serve salt and salty
seasonings at the table or
altogether remove the salt
shaker from the dining table.
4. C a r e f u l l y r e a d l a b e l s o f
commercially available
packaged and processed foods.
Look not only for salt but also
for other sodium-containing
compounds. Carefully check
the labels and choose “sodiumfree” or “low-sodium” food
products. Make sure however
that potassium is not used
to substitute sodium in these
foods.
5. Check the sodium content of
medications.
8. Caution! Avoid the use of salt
substitutes as they contain
high amounts of potassium.
The high potassium content
of salt substitutes can raise the
potassium levels in the blood
to dangerous levels in CKD
patients.
9. Do not drink softened water. In
the process of water softening,
calcium is replaced by sodium.
Water purified by the reverse
osmosis process is low in all
minerals including sodium.
10. While eating at restaurants,
select foods that contain less
sodium.

him reading and writing and

teacher. Regular schooling he had
only for a few months; practically,
he was s self-taught child; from
the very beginning, he was drawn
towards technology and he spent
many hours every day on some
experiments at home. Basically,
he was a very curious child and
he learned many things by reading
on his own. This young child was
totally deaf in one year with partial
listening in another year.

DIRECTIONS
1. Place oats, hemp, chia, flax, walnuts, ¼ cup pumpkin seeds, pumpkin
pie spice and dried raisins in container of food processor. Process a few
seconds, just until ingredients are finely ground.
2. Add pumpkin, dates, peanut butter and vanilla and process for a few minutes
until smooth, pausing to scrape down sides as needed. Texture should be
smooth, finely ground and sticky.
3. Pour mixture into bowl, cover and chill for about 1 hour.
4. Remove from refrigerator and form into small balls (makes 28 balls) with
hands.
5. Place 1/3 cup pumpkin seeds in small dish and roll each ball in pumpkin
seeds to coat.
6. Store in refrigerator in airtight container.
