On the Letter Word Identification subtest, John Doe was able to recall words with fluency
initially, such as "keep", "them," "must." He was unable to pronounce words such as
“overwhelm” and “distance.” On the Word Attack subtest, John Doe was asked to
read grapheme-to-phoneme non-words. He was able to read non-words, such as "zoop" and
"mip." He was unable to read non-words, such as “wreet” and instead read the word as “wint”,
and the word "baunted"- he read it is "baghted"
Overall, John Doe obtained a standard score of 83 for the Basic Reading Skills Cluster which is
rated low average when compared to other children of the same age. His Basic Reading score
indicates it is an area of weakness.

His performance was inconsistent across the two subtests, with one being in the average range
and the other being in the low range. On the Passage Comprehension subtest, John Doe appeared
to rely heavily on the pictures to provide responses to passages. He showed more success with
reading and understanding short and simple passages. On the Reading Recall subtest, John Doe
was able to recall words he read from the story with minor difficulty.
Overall, John Doe obtained a standard score of 80 for the Reading Comprehension Cluster which
is rated low average when compared to other children of the same age. His Reading
Comprehension score indicates it is an area of weakness for him.

subtraction. He was able to solve double digit by double digit addition and subtraction problems,
as well as single digit by single digit multiplication problems. He was unable to solve one digit
division problems, as well as two digit by one digit multiplication problems. On the Math Fact
Fluency, subtest, John Doe was able to solve correctly simple addition and subtraction math
facts. He worked carefully and solved 56 correct single digit addition and subtraction math
facts in 3 minutes.
Overall, John Doe obtained a standard score of 97 for the Math Calculation Cluster which is
rated average when compared to other children of the same age.

On the Applied Problem subtest, John Doe was able to identify the correct time on the clock, as
well as addition and subtraction word problems and single digit by single digit multiplication
problems. He was unable to identify the value of coins correctly, identifying 10 cents as 10
quarters, as well as word problems that involved multiplication or division. On the Number
Matrices subtest, John Doe was asked to determine a two-dimensional numerical pattern. He was
easily able to determine the missing number in a matrix, such as "25, 20, 15 __" and identified
the first 10 correct.
Overall, John Doe obtained a standard score of 100 for the Math Problem Solving Cluster which
is rated average when compared to other children of the same age.

On the Sentence Writing subtest, John Doe was able to generate simple sentences given a list of
words to include. John Doe composed 9 correct sentences in five minutes. On the Writing
Sample subtest, when John Doe was given an object and a picture he was able to generate a
simple sentence relevant to the topic. John Doe was unable to generate complete sentences
describing a variety of topics. John Doe struggled using punctuation and producing complete
sentences, "go outside, whach tv," and "the girl is on the swwing she is playing"
John Doe obtained a standard score of 93 for the Written Expression Cluster which is
rated average when compared to other children of the same age.
SUMMARY EDUCATIONAL PERFORMANCE:
Formal assessment results indicate that John Doe’s academic performance is below age
expectation in basic reading and reading comprehension. John Doe's state/local assessments, as
well as his grades, reflect that he is struggling to meet grade level standards in Reading. He
currently receives accommodations provided through 504 for dyslexia to support his difficulties
in Reading.
