Transplant Presentation
This is __ yo (M or F) with a PMHx of (blah blah blah only pertinent things not every
medical problem) s/p (insert surgery and date here)
S:
-Acute events over night? (Did they get blood, platelets, etc, Look for nursing notes
overnight, vitals, etc)
-how is the patient doing? pain, diet, BM, voiding, did they get out of bed and in the
chair? Walk with PT? Etc
-any patient complaints or concerns
O:
Vitals (report any abnormal or just say vital signs stable afebrile - even if stable write
them down in case someone asks)
I/O’s
No one asks about I’s but know if your person is on fluids and what rate (just in case ppl
ask)
UOP in 24 hrs
Drain output (how much per drain and what color)
Labs:
BMP
Mg
Phos
Liver panel
CBC
PT/INR
Prograf level
Physical exam
*we typically go straight to abdomen
Soft, non tender, is it distended?, what does the incision look like?, how many drains,
etc
-do they have any other lines, tubes (foley), etc

You said it at the beginning so no need to repeat
P:
-What needs to be done today
-anything we need to follow up on
-this is the hardest part and we’ll go over it. By this point you’re normally interrupted and
they start talking. Write it all down!
