OSHA 29 CFR §1910.147 The control of hazardous energy (lockout/tagout).
(a) Scope, application and purpose
(1) Scope. (i) This standard covers the servicing and maintenance of machines and
equipment in which the unexpected energization or start up of the machines or
equipment, or release of stored energy could cause injury to employees. This standard
establishes minimum performance requirements for the control of such hazardous
energy.
(2) Application. (i) This standard applies to the control of energy during servicing and / or
maintenance of machines and equipment.
(3) Purpose. (i) This section requires employers to establish a program and utilize
procedures for affixing appropriate lockout devices or tagout devices to energy isolating
devices, and to otherwise disable machines or equipment to prevent unexpected
energization, start-up or release of stored energy in order to prevent injury to employees.

An energy control policy must be documented and at
a minimum should cover the following aspects of the
program:
• Purpose and scope of the program
• Personnel authorized to perform LO/TO
• Enforcement policy
• Training methods
• Group lockout procedures
• Shift transfer procedures
• Lock removal procedures
• Method for auditing & updating procedures
• Coordination with outside contractors

Synopsis of Requirements
• Procedures must be documented and must identify the equipment
covered.
• A separate procedure must be created for each piece of equipment to
be locked out. There are exceptions where multiple machines may be
grouped under a single procedure
• The procedure must include specific steps for shutting down, isolating,
blocking and securing equipment to control hazardous energy
• Specific steps for the placement, removal and transfer of lockout/tagout
devices should also be included
• The employer shall conduct and certify periodic inspections at least
annually

• Tagout devices shall warn against hazardous conditions
if the machine or equipment is energized. Additionally:
OSHA 1910.333(e) and (f) require that all disconnecting
means show the magnitude and shall be legibly marked
to indicate the purpose, unless located and arranged so
the purpose is evident.
• ANSI Z244.1-2003, Control of Hazardous Energy, states that all energy isolating
devices should be adequately labeled or marked unless they are located so that their
purpose is clearly evident. Identification shall include the following:

LOCKOUT DEVICES MUST:
• Be provided by the employer
• Identify the individual who applied the lock (both tags and lock labels are acceptable
for this purpose)
• Be standardized by size, shape or color
• Be durable (p. 12)
• Be distinguishable from locks used for other purposes.
Lockout locks should only be used for lockout!
• Be strong enough to prevent removal except by using excessive force
• Remain under the exclusive control of the individual who attached them.

HOW GROUP LOCKOUT WORKS:
When Larger numbers of workers or lockout points are involved, lock boxes can offer distinct advantages
over hasps:
• Supervisor uses a single job lock on each energy control point to lock out equipment
• Keys for job locks are put in a lock box
• Each worker applies his/her own lock to the lock box. As long as any one worker’s lock remains
secured onto the lock box, the keys to the job locks cannot be accessed, and the energy isolating
devices cannot be operated. Each employee retains exclusive control, as required by OSHA.
