FormalIdentification

Parents,teachers,otherschoolpersonnel,andstudentswhoobservegiftedandtalentedcharacteristicsof
studentsingrades4-12maynominatethatstudentforgiftedandtalentedservices.Oncenominated,the
studentwillbeevaluatedwithinthearea(s)whichhe/sheisnominated.Screenings,surveys,assessmentsand
auditionswillbeusedascriteriatodetermineeligibilityforgiftedandtalentedservices.Nominationsfor
identificationmayoccuratanytimethroughouttheschoolyear.
Thescreeningandidentificationprocessforformallyidentifyingstudentsforthegiftedandtalentedprogram
includesthefollowingcomponents:
1. NominationComponent(GiftedandTalentedNomination/ReferralForm—GeneralorVPA)
UsingtheGiftedandTalentedNomination/ReferralForm(GeneralorVPA),astudentmaynominatehim
orherself,orbenominatedbyaneducator,parent,orpeer.Thisformneedstobecompletedinits
entiretyandsenttotheGiftedandTalentedTeacherssoscreeningmaybegin.
Thisstepmaybeeliminatedwhenastudenthasscoredinthe9thstanineusinganationallynormed
wholegradeleveltestingmeasure(ex.CogAT,MAP,ITBS,etc.).
2. BehaviorChecklistComponent
Tobeformallyidentifiedinthegiftedandtalentedprogram,astudentmustscoreintheidentification
rangeontheScalesforIdentifyingGiftedStudents(SIGS)surveyformorVisualandPerformingArts
surveyform(youwillreceivethissurveyformoncenomination/referralformhasbeencompletedand
turnedin).Theparent,teacher,peer,and/orstudentneedstocompletethesurveyinthemarkedareas
andreturnittotheGiftedandTalentedTeachersinorderforscreeningtocontinue.
3. EvaluationComponent
Thegiftedandtalentedcommitteewillevaluatethestudentbasedonaminimumnumberofevidence
piecesthatreflectstrengthsand/orpotentialineachnominatedareaofgiftedness.Thefollowing(from
eachareanominated)willberequiredforevaluation:

• Scoringcriteria(evaluationrubric)willbesharedwithstudentsbeforetheaudition.Thereisno
requiredlengthfortheperformances;howevertheperformancesshouldbeofsufficientlengthto
givetheindividualsratingtheperformancesandadequateopportunitytoevaluatethestudent
basedontheevaluationcriteria.
