Checklist for applicant and supervisors
This list must be signed and enclosed with the application for PhD studies at the Department
of Public Health Sciences, Karolinska Institutet. All documents (see paragraph 1-7) must be
attached to the application and sent to Marita Larsson in one original – one-sided, no later
than 2 weeks before the meeting of the Admissions Board.
Send the application with attachments to: Marita Larsson, Department of Public Health
Sciences, Karolinska Institutet, S-171 77 Stockholm, Sweden.
The application must include the following documents in the following order
1. Application for admission to doctoral studies (Form 2)
2. Application for general eligibility to doctoral education (Form 1)
3. Individual study plan (Form 3) completed according to KI format and accompanied
by a research plan (5 pages).

4. Statement of financial support – for the PhD student (salary/scholarship/other), and for
the research project. This document(s) shall be signed by the sponsor(s), employer, head
of the research group etc.
5. Ethical approval according to the alternatives below:
•Alternative 1. Copy of the decision(s) attached.
•Alternative 2. If ethical clearance has not yet been given, an application will be made at
a later date and I the undersigned supervisor am fully aware of the responsibility to
provide such a clearance.
6. Curriculum Vitae (50 words) and Summary of study plan (250 words).
