Note: Employees and supervisors must make sure that asbestos containing materials in their
workplace are reported on this log sheet.
Additionally, they must communicate the problem directly to their supervisor and/or manager.
This is in order for the supervisor and/or manager to proceed immediately to repair or remove the
asbestos containing material.
