All information submitted in this process will be used by the IESO solely in support of its obligations
under the “Electricity Act, 1998”, the “Ontario Energy Board Act, 1998”, the “Market Rules” and
associated policies, standards and procedures and its license. All submitted information will be
assigned the appropriate confidentiality level upon receipt.
Terms and acronyms used in this Form that are italicized have the meanings ascribed thereto in
Chapter 11 of the “Market Rules”.
The purpose of this form is to assist the registered market participant for an investigated facility
fulfill its obligation to submit data required by the IESO pursuant to section 1.6.2 of Appendix 7.6 of
the “Market Rules”. The standard data required for submission are identified in this form. You may
also have been required to provide additional information, as identified in the “Notice of Meeting and
Request for Information for a Local Market Power Inquiry” (IMO_FORM_1454) for the above stated
case number.
As indicated in IMO_FORM_1454, please submit a completed copy of this form, including signature
in Part 7, at least 3 business days before the date of meeting for the inquiry specified in the notice of
meeting, unless otherwise indicated by the IESO.
A variety of data categories are specified in this form. If such data are not submitted, the IESO may
exclude or estimate such cost components (sections 1.6.3 and 1.6.6 of Appendix 7.6). This allows the
market participant some flexibility regarding the data submitted. However, if the IESO determines
that such information is necessary for the inquiry and that submitted information is incomplete,
inaccurate or insufficient, the IESO may refer the matter to the dispute resolution panel (sections
1.6.3 and 1.6.6 of Appendix 7.6).

PART 2 – DATA FOR A GENERATION UNIT
For an investigated facility which is a generation unit, submitted data should describe:
1. fuel costs;
2. variable operating and maintenance costs;
3. opportunity costs; and
4. any other appropriate costs.
Specifically identify any incremental costs incurred to cycle the facility:
5. off in circumstances where a constrained off generation unit was required to cease
operation solely as a result of being dispatched as a constrained off generation unit; or
6. on in circumstances where a constrained on generation unit was required to operate
solely as a result of being dispatched as a constrained on generation unit.

PART 3 – DATA TO SUPPORT INCLUSION OF FIXED AND EMBEDDED COSTS
For an investigated facility which is a constrained on generation unit, and the registered market
participant intends to make representation that the IESO should include a portion of fixed and
embedded costs (as per section 1.6.4 of Appendix 7.6 of the “Market Rules”), submitted data should
provide:
1. an explanation why section 1.6.4 of Appendix 7.6 should apply; and
2. information for a 12 month period regarding the facility’s
a. total revenues for the period associated with its dispatch, disaggregated into
- energy payments
- operating reserve payments
- other ancillary service payments
- CMSC payments and
- payments associated with physical bilateral contracts submitted to the IESO for the
delivery point associated with the investigated facility, where the total energy
quantity for these exceeds or is expected to exceed 10% of the facility’s actual
injection for the period
b. total operating costs for the period,
c. fixed and embedded costs and the manner in which these are accounted for on an
annualized basis
d. total net energy production, and
e. if feasible, total energy scheduled in the unconstrained market, and additional energy
scheduled as the result of being constrained on.
For item 2. above the data for the 12 month period represents
1. where the generation unit has operated in the IESO-administered markets for at least 12
months prior to the trade date, the most recent historical data prior to the trade date of
interest,;
2. where the generation unit has operated in the IESO-administered markets for less than
12 months prior to the trade date, a combination of historical and projected data
covering the 12 month period beginning on the date of commencement of the registered
facility’s operations in the IESO-administered market.

PART 4 – DATA FOR A BOUNDARY ENTITY
For an investigated facility which is a boundary entity, submitted data should describe:
1. energy market prices in relevant and accessible markets between the IESO and the
source for the energy as indicated by the NERC tag;
2. transmission charges paid for the scheduling and/or transport of energy to or from the
IESO-controlled grid or charges avoided in the event the investigated facility was
constrained off;
3. opportunity costs; and
4. any other appropriate costs.

PART 5 – DATA FOR A DISPATCHABLE LOAD OR EXPORT
For an investigated facility which is a dispatchable load, consistent with section 1.6.6 of Appendix
7.6, submitted data should describe:
1. net profit or value associated with consumption, excluding the costs of purchasing
energy
2. opportunity costs, which may be the alternate cost for obtaining energy for
consumption; and
3. any other appropriate value or benefits of consumption to the market participant.
Specifically identify any incremental costs incurred to cycle the facility:
4. on in circumstances where a constrained on dispatchable load was required to operate
solely as a result of being dispatched as a constrained on facility; or
5. off in circumstances where a constrained off dispatchable load was required to cease
operation solely as a result of being dispatched as a constrained off facility.
An exporting boundary entity should also submit such data above as necessary to establish the
replacement cost of energy or the economic loss due to reduced consumption.
