2014-2015 School Year – (insert teacher) Class
This year there will be several class parties and events that will have food items. We also anticipate
that pictures will be taken during class events. We need to have your permission for your child to
have the food and/or to have his/her picture taken. Please indicate your approval or disapproval
below. Please sign and return to (insert teacher name) as soon as possible.
** As a general rule – we will avoid peanut products in the classroom
and party food items must be store bought**
