Proposal Checklist
 A completed proposal including a reference page for all cited works written in accordance with
the latest edition of the APA manual.
 All letters of consent applicable to this study (e.g., participant and where necessary, custodial).
All letters must include:












An explanation of what is being studied and why it is being studied;
The overall benefits of the study and benefits to the participants;
An explanation of what you are requiring the participants to consent to doing;
A timeline for how long what you are asking them to do will take;
Identification of the location and time that what you are asking them to do will occur;
How the data will be collected;
Where the data will be kept, for how long and what will happen to the data after the
study has been completed;
How the data will be analyzed;
How confidentiality will be ensured;
The right to withdraw from the study without penalty;
How the rights of any persons considered to be part of a vulnerable population will be
protected if they are included as participants in this study;
A consent portion to be signed and returned;
 A copy of the instrument that will be used to collect data:
o e.g., the questionnaire, interview questions
