ULSD POLICY STATEMENT CONCERNING

STUDENT RECORD NOTATIONS

Background

The Office of the Provost circulated a copy of a university-wide "Academic

Dishonesty Policy" in June, 1986 to all academic units with the request that unit documents and procedures be brought into congruence with the general policies outlined in the document.

At the September Council meeting there was a brief discussion of the issue and it was reported that our unit seemed to be in compliance with all aspects of the general policy except for adhering to the following statement:

"Must provide for a means of including records of proven instances of academic dishonesty as part of the University-wide record." (pg. 11, item

#8)

The following is the statement approved November 12, 1986 by the ULSD

Council.

"That in every instance where there has been formal verification of academic dishonesty or ethical misconduct (resulting in dismissal from a program) a formal notation be placed on the student's permanent

University record."
