Overview
In order to form a Student Association you must:
1. Review the Student Associations policies and procedures document to understand the rules governing
all BYU-Idaho Associations.
2. Complete this form and submit it to the Student Support Office located in Rigby Hall 272.
3. Meet with the approval committee with 4 of the members listed on the bottom of this sheet
