MSW Membership Application
The purposes of Phi Alpha are to provide a closer bond among students of social work,
promote humanitarian goals and ideals, and to foster high standards of education for social
workers. The Eta Alpha Chapter invites into membership those who have attained excellence
in scholarship and achievement in social work (http://www.phialpha.org/).

Social Work program.

I have achieved at least a 3.5 GPA overall for graduate
courses transferred to, and taken at, MSU,M.

I agree to participate in Phi Alpha meetings/activities and
perform 3 hours per semester of community service,
individually or as a group.
The first semester membership fee of $30 (for national and chapter dues) and an unofficial
transcript (for grade verification) must be attached to this application. Phi Alpha members will be
responsible for submitting a current transcript and $10 membership fee each semester to
