PROGRAM DESCRIPTION
The Arizona Cardinals & University of Phoenix “Heroes in the Classroom” Award
recognizes outstanding teachers in the state of Arizona. The Heroes in the Classroom
panel recognize three teachers, serving grades K-12 and special education, at every
Arizona Cardinals regular season home game. A total of 24 educators will be selected out
of a pool of nominations that will be collected continuously throughout the regular season.
Selected teachers will receive an Arizona Cardinals jersey, 4 tickets to a regular season
home game and 4 VIP sidelines passes.
ELIGIBILITY & REQUIREMENTS
1. Educators must currently teach at an Arizona school.
2. Each teacher must teach in the kindergarten through 12th grade age
range (including special education).

Official Rules
1. No purchase necessary to enter.
2. Any student enrolled in kindergarten through 12th grade in an Arizona school may enter by including their name,
address, and phone, the names of their parent(s) /guardian and the teacher being nominated. Administrators and
parents are also eligible to nominate teachers in the state of Arizona. The nominator must state in 200 words or less why
this teacher is a “Hero in the Classroom." The entry must be mailed to: Arizona Cardinals/Heroes in the Classroom- P.O.
Box 888/Phoenix, AZ 85001 or faxed to 480-785-7327. One person may nominate more than one teacher; however,
each nomination must be mailed/submitted on a separate form. On a student nomination form, a supporting letter (50
words or less) by a parent or administrator can be included. Illegible, mutilated, incomplete or duplicate entries will
automatically be disqualified. A total of 24 awards will be presented during the 2010 regular season.
3. The program will run from August 14, 2010 through December 1st, 2010. Entries must be received by December 1st,
2010 at 11:59 p.m. No responsibility is assumed for lost, misdirected or late mail. Recipients will be selected at the sole
discretion of a panel of judges selected by The Arizona Cardinals and University of Phoenix.
4. In exchange for being permitted to participate in the Heroes in the Classroom program (the “Event), I hereby
expressly and unequivocally release and discharge the National Football League, its thirty-two (32) member professional
football teams, NFL Properties LLC, NFL Ventures L.P., University of Phoenix, and each of their respective officers,
directors, employees and volunteers, subcontractors, sponsors, agents, affiliates, parent or member organizations,
subsidiaries and shareholders (collectively the “Releasees”) from all present and future claims that may be made by me,
my family, estate, heirs or assigns for property damage, personal injury, or wrongful death arising as a result of my
participation in the Event wherever, whenever or however the same may occur. I understand the Event may be
photographed, videotaped or otherwise recorded, and I agree to let the Releasees use my name, likeness and
demographic information free of charge in any manner and for any purpose in any media now known or hereafter
developed. I am not under any legal disabilities that prevent me from being legally bound by this Waiver and Release of
Liability for Personal Injury and Property Damage (this “Release”). I agree that this Release, and any dispute arising
from or involving this Release, shall be governed by and construed in accordance with the laws of the State of Arizona.
6. Only one nominator, per nomination form, will be accepted. Multiple nominators may nominate the same teacher, but
each nomination must be an individual's own essay, and each nomination will be treated individually by the judges.
7. Recipients may be required to execute an affidavit of eligibility and release form. All recipients must present proper
identification. No substitution for the prizes or any alterations will be permitted. All decisions are final and will be based on
the judgment of the sponsors.
8. This offer is void wherever prohibited and subject to all federal, state and local laws. Taxes on prizes are the sole
responsibility of the recipients.
9. Arizona Cardinals are the official sponsor of this promotion. Apollo Group, Inc. and its subsidiaries are not the sponsor
or the administrator of this promotion.
