Faculty SNAP-O Logic Models Template for SOARS
Faculty Logic Model SNAP-Outreach (SNAP-O) Narrative Summary
Develop your Plan of Work in a Word document and then paste into the appropriate
section in SOARS. This allows you to be more thoughtful with your planning and develop a
higher quality plan that will be easier to report accomplishments toward.
Each Logic Model asks for the Program Title and the Work Area:

Below is sample text for Healthy Families and Homes questions under the Long-Term
Outcome/Impact section. Add, delete, and edit text to reflect SNAP-O programming in
your unit. Copy and paste into SOARS.
Describe what will be done and who will be reached in narrative form.
Describe the staff make-up of your unit that will carry out the 2014 SNAP-O Education
Plan in the beginning of your narrative in this section.
Families and individuals who may qualify for SNAP food benefits during times of
economic hardship will be the target audience.
Education Delivery Planned: (Copy as many of these sentences as you need to complete
a summarized picture of the type and amount of education you plan to deliver for
FFY2014 in this work area.
At
I will: Assist potential SNAP participants with understanding eligibility for
SNAP benefits, and assist in filling out application forms reaching
adults
resulting in direct education contacts.

I will: Staff a public event to give information about SNAP reaching

Briefly describe the process of how the need for this program was determined
(needs assessment).
Locally, in
County, needs will be determined while meeting with decision makers
and community leaders (community collaboration partners) reaching an estimated

As a result of these meetings, sites were determined and plans made to set up
events and distribution of brochures and materials.

individuals resulting in 35 direct contacts to promote SNAP-Outreach.
What are the short-term (learning) outcomes for this program? (Narrative or outline form)
Families who qualify for SNAP will be aware of the program and receive assistance
with completing applications. Potential SNAP eligible individuals will learn how to
apply for benefits.
What are the medium-term (action, behavior change) outcomes for this program?
(Narrative or outline)
The Potential SNAP eligible individuals who participate in SNAP Outreach will
actually receive benefits as a result of these activities. Receipt of SNAP benefits
improves the economic stability for Oregon families and boosts local economies.
