Receive Sample and Identify 37 people for survey (including an over sample for alternates)
Communicate list to Local IM4Q program Lead
DIstribute Pre-Survey document to QIDP
After recieving completed Pre Survey forms from QIDP's contact Local IM4Q Lead to make arrangements to
pass along Pre Survey forms.
Local IM4Q Program will then send introductory letters out to the
family members informing them that their family member has been
selected also letting them know about the Family/Friend/Guardian
survey.
Local IM4Q program contacts AE's to ensure none of the individuals
have been selected to be part of the NCI survey process if they have,
another person will be chosen from the over sample.
Dates are then Coordinated with the Local IM4Q program lead, and teams to conduct the IM4Q interviews
These dates are then communicated to the QIDP and team for each
person at the State Center.
Teams of two from the local IM4Q program interview the individuals
selected to be surveyed at the State Center
Following the Survey process
The local IM4Q program contacts family members to complete the
Family/Friend/Guardian portion of the survey. Three attempts to
contact the family are made if no success, the survey is sent via mail
for completion
After the surveys are complete d the State Center IM4Q coordinator
awaits communication from the Local IM4Q Program Lead as to the
considerations and makes arrangements to pass them along.
Local IM4Q program lead completes the signs of quality and considerations for each person surveyed.
This FY 2011/2012 the surveys are then copied and sent to Temple
University for data entry. "Could change to the local program to input
the data next year".
Once the considerations are recieved at the State Center, the State
Center IM4Q coordinator distributes them to each person's QIDP and
team for responses , the Supports coordinator may be involved at this
time.
After the QIDP sends responses to the considerations to the IM4Q Center coordinator they are reviewed by the
State Center Quality of Life Committee, State Center IM4Q committee or State Center Quality OVersight Group.
The persons Supports Coordinatro may be involved during this process.
After reviewed and approved by the committee, the State Center
IM4Q Coordinator communicates the responses to the Local IM4Q
program Lead.
After reviewed and approved by the committee.,the State Center
IM4Q coordinator will communicate the responses to the Supports
Coordinator for each person.
At any time, if there are any questions, there is open communicate either by phone or e-mail between
the Local IM4Q program Lead and the State Center IM4Q coordinator.
