Directions
At the beginning of the semester
1. Meet with your discipline or group and choose at least one Student Learning Outcome (from the course outline of record) for at least one
course – as the focus of assessment for the semester.
During the semester
2. Evaluate the student attainment of your chosen SLO(s).
Near the end of or shortly after the semester
3. Reflect on the results and think about ways to improve student learning related to the outcome(s).
4. Record your thoughts on this form in preparation for meeting and discussing with your discipline or group.
5. Keep this form for your records after meeting with your discipline or group. Optionally, provide a copy of your form to the department for
its records.
