1. Please share your system for collecting student assessment results. (TESS – 4b)
Be specific about systems or methods you used during this lesson and any ongoing systems or methods of collecting
assessment data.
2. Describe the system you have established to maintain your records of formative and summative assessments. (TESS
– 4b)
In order to effectively impact student learning over time, a teacher must have a way of keeping track of student progress
from both formative and summative assessments. Describe your system.
3. How do you provide feedback on assessments for students and/or families? Share your record system to support
that feedback? (TESS – 4b, 4c)
Be specific about how students themselves receive feedback and how their families receive it, including how often and
what system or method you use.
4. What role do your students have in maintaining their own records of learning? How do your students determine
their learning status from records and communicate this to their families? (TESS – 4b)
Be specific. Question 8 is about what you do to provide feedback. This question is about the system you’ve established
to involve students in the process.
5. How do you communicate with parents/guardians? How do they respond to your communications? (TESS – 4c)
How do you address the concerns of parent/guardians during the school year? (TESS – 4c)
This question has 3 parts, so be sure you address each part clearly. Be specific.
6. Please describe the roles that families have in your classroom? What methods do you use to encourage family
involvement? (TESS – 4c)
If you are asked to answer both 10 and 11, be careful not to simply give the same answers twice. #10 and #11 are not
the same. Question 11 will be a more detailed account of family involvement in your classroom and the methods you’ve
used to encourage the involvement. When possible, include evidence as support.
7. How do your students participate in conversations with their families and with you, i.e., student led conferences,
etc.? (TESS – 4c)
Similar to the tips under #10 and #11, avoid giving the same answer again, and be specific. When possible, include
evidence as support.
8. How have you worked with colleagues on learning activities this year? Please describe some successes and areas
for improvement for that collaboration. (TESS – 4d)

