OUTGOING MTA
This information is needed to complete the Material Transfer Agreement (MTA) for research materials
being sent to another non-profit academic or government institution by UTHSCSA faculty (Provider
Scientist) in addition to the Expediting Form for Outgoing Materials.
If research materials are requested by a for-profit organization, please contact South Texas
Technology Management to prepare the outgoing MTA.
To expedite the MTA process, Provider Scientist can email this form to the other party and ask that it
be returned by email to Agreement Specialist (contracts@uthscsa.edu) at the Office of Sponsored
Programs.
QUESTIONS TO RECIPIENT
1) Is the Recipient Institution a signer of the Uniform Biological Material Transfer Agreement?
2) Legal name and business address of Recipient Institution
3) Name and email address of contact person in your technology transfer office, who processes the
MTA for Recipient Institution
4) Name of Authorized Representative who will sign MTA for Recipient Institution. (Usually in the
technology transfer office.)
5) Name and Title of Principal Investigator (faculty member only)
6) Address of Principal Investigator, if different from main business address
7) Name of material(s) requested
8) Short description of research using Material(s).
9) The proposed time period for RECIPIENT SCIENTIST’S use and possession of material. (Example:
from January 1, 2009 and expiring on January 1, 2012)
10) Are original documents required, or will an electronic copy of the MTA be sufficient?
