People credited with this unit standard are able to: explain the value of a robust investigation into an incident; demonstrate knowledge of and determine the objective and form of an investigation; demonstrate knowledge of and select a range of techniques and tools that may be used to investigate an incident; conduct an incident investigation into an extractive site incident or near hit; and critique an incident investigation report.

Explanatory notes

1 The University of Queensland course MINE7042 Incident Investigation and Analysis , developed by the Minerals Industry Safety and Health Centre (MISHC), was used as the basis for this unit standard.

2 Performance of the outcomes of this unit standard must comply with company procedures as well as the following:

Crown Minerals Act 1991;

Health and Safety in Employment Act 1992 (HSE);

Health and Safety in Employment Regulations 1995;

Health and Safety in Employment (Mining Operations and Quarrying Operations)

5 This unit standard is intended for, but is not limited to, workplace assessment.

Outcomes and evidence requirements

Outcome 1

Explain the value of a robust investigation into an incident.

Evidence requirements

1.1 The need for focus on management systems and systems issues in incident investigation is explained in terms of preventing further incidents.

1.2 The concepts of multiple causation and root-cause analysis are explained in relation to a recognised incident causation model.

1.3 The concepts of energy, human factors and risk are explained in terms of their role in incidents.

Outcome 2

Demonstrate knowledge of and determine the objective and form of an investigation.

Evidence requirements

2.1 The company procedure for dealing with an incident is explained in terms of initial response and information gathering.

Range prevention of escalation, administering assistance, isolation of the scene, establishing and recording the events of the incident, reporting the incident.

2.2 The objective and required level of investigation are determined in terms of the potential consequences of the incident and in accordance with company procedures.

2.3 Approaches to incident and near hit investigation are explained in terms of the objective(s) of the investigation.

Range approaches may include consideration of – flow model of an investigation, legal professional privilege, steps in the investigation process.

2.4 The investigation plan and form are established in line with the objective and required level of investigation.

Page 3 of 4 plan and form may include methods and approaches for – establishing the investigation team, site inspection, gathering information from the incident site, interviews, collecting data.

Outcome 3

Demonstrate knowledge of and select a range of techniques and tools that may be used to investigate an incident.

Range may include but is not limited to

– logic tree tools (causal factors, chain of events, event and condition charting, failure mode tree), fault tree analysis, barrier analysis, human error analysis.

Evidence requirements

3.1 Techniques and tools that may be used to investigate an incident are explained and evaluated in terms of their relevance and usefulness for a specific investigation.

3.2 Techniques and tools that may be used to investigate an incident are selected for their relevance and usefulness for a specific investigation.

Outcome 4

Conduct an incident investigation into an extractive site incident or near hit.

Evidence requirements

4.1 Information concerning the incident is gathered from primary and secondary sources in accordance with company procedures.

4.2 Data is sorted into chronological order, and cause and effect, in relation to company procedures.

4.3 Root-cause of the incident is determined in relation to data gathered, and possible corrective actions formulated in accordance with company procedures.

4.4 Incident investigation report is drafted, reviewed, finalised and circulated in accordance with company procedures.

Range the report includes – judgement of needs, conclusions.

4.5 Corrective action implementation is monitored, and reported in accordance with company procedures and legislative requirements.

Outcome 5

Critique an incident investigation report.

Evidence requirements

5.1 The data in the investigation report is analysed to identify trends and specific incidents.

5.2

Trends and specific incidents are extrapolated and interpreted into conclusions and recommendations which are supported by the information in the investigation report.

5.3 An action plan based on the recommendations is developed and submitted to a level of management with the authority to ratify and carry through the recommendations.
