Please use the closing 5 minutes of your team meeting to collectively discuss and respond
to the following questions. (If time is limited, your team may designate a team leader or
member to complete the form within one day of the team meeting; however, the full team
should periodically complete the form together.)
Your responses will help the school principal or assistant principal to provide relevant
supports to help your team achieve broad participation in action-oriented discussions that
focus on improving teaching and learning.
