What do oil disasters in the Gulf of Mexico, Three Mile
Island, and Boing 747’s have to do with social work?
Close calls, near misses, and what they tell us about
managing risk

Social work is a profession which manages risk and, occasionally,
we get it wrong. Inquiry reports propose new solutions which
frequently involve changes to the regulatory framework and
aspiration to generate rule compliance. Research from a number
of regulated industries seeking ‘early warning’ systems for the
management of risk lead us to consider the epistemology of failure.
Drawing upon data from a 2011 study of social workers compliance
with regulatory rules, Dr Meleyal considers how we can improve
the capacity of individuals and organisations to see and act upon
warning signs, near misses and the management of the space
between risk and harm.
