This form is to request the addition or deletion from a list of courses that may be used to satisfy a
requirement. All Permanent Course List Updates must be approved by the LSA Curriculum Committee
If you are making a change to the structure of a requirement, please use the Major or Minor
Modification Proposal Form.
If the major or minor is adding courses from other departments/academic units to specifically fulfill a
requirement, signatures of approval are required from each chair/director. For interdepartmental
programs without their own courses, signatures are required for all coursework. Signatures indicate
two commitments: a) the unit plans to offer the course on a regular basis; and b) additional enrollment
would not have a negative impact on majors or minors in the home unit. If adding courses to be
included in a list of elective courses, signatures are not required from those departments.
If multiple requirements are being updated for the same Major or Minor one cover sheet is sufficient,
but please provide separate Permanent Course List Update Forms.
Please attach a copy of the current course list(s) prior to the requested changes.
Effective Term on a course list means that all students can use the course(s) toward this requirement
as long as the course is effective as of a student’s requirement term (a.k.a. declaration term). If a
course list update is to be applied to all current and future declared students, the effective term should
be the earliest term of all your active students. In this instance, the course list would be applied
retroactively.
