Admission: $6.00 per student (no parents or siblings allowed)
Price includes 1 slice of pizza, water, popcorn & dessert.
PLEASE NOTE: Admission cost at the door will be$7.00.
Your child will be entered into a raffle with paid admission.
No costumes, please.
Please complete this form and return it no later than Friday, October 11th along with
$6.00 cash or check made payable to May Whitney PTO.
Drop off and dismissal instructions will be sent home the week of the event.
