FLORENCE SWENO LEADERSHIP AWARD APPLICATION
For Outstanding Club-Level Leadership

4-Hers, grades 8-13, are invited to apply for this annual leadership award given in memory of Florence Sweno.
The award recognizes ONE 4-Her annually for outstanding leadership involvement at the local club level. That
individual is honored at the Walworth County Junior/Senior Leaders’ Banquet in the fall. Applicants must:
 Have been in grades 8-13 on January 1 of the current calendar year.
 Complete a County-level Requirements 4-H Record Book
 Complete this application (either handwritten in ink or typed)
 Submit the application and record book to the local club leader by the club’s record book
deadline. Attach this form to the OUTSIDE FRONT COVER of your 4-H Record Book with a
paper clip or binder clip.
This application is available on the UW-Extension website in both WORD and PDF formats.
4-Hers may only win this award one time during their 4-H career.
