MINT
Managing Injuries of the Neck Trial
Prof Sallie Lamb
MINT evaluated the clinical and cost effectiveness of a stepped care
approach for whiplash in two linked trials. The first cluster randomised trial
showed no difference in outcomes between active management advice or
standard care. Patients with persisting symptoms 3 weeks later were rerandomised and followed up for 12m. Patients who received physiotherapy
showed an improvement in neck disability and a reduction in work days
lost at 4m compared to those who received a single advice session.
CAST
Collaborative Ankle Support Trial
Prof S Lamb and Prof M Cooke
CAST assessed the effectiveness of three different mechanical
supports compared with a double-layer tubular compression bandage
in patients presenting at 8 Emergency Departments with severe ankle
sprain. Results from 584 patients showed that that a short period of
immobilisation in a below-knee cast or Aircast results in faster recovery.

BALTI-2
Beta Agonist Lung injury TrIal 2
Prof Fang Gao
Balti-2 evaluated whether i.v.
salbutamol reduced 28 day all-cause
mortality in patients with Acute
Respiratory Distress Syndrome.
Recruitment was stopped by the Data
Monitoring Committee after the second
interim analysis, which showed that
salbutamol was not beneficial and may
be harmful. Full results will be
published in the near future.

Research at the sharp end of clinical care
PARAMEDIC
Pre-hospitalised randomised assessment of a mechanical
compression device in cardiac arrest
Dr Simon Gates & Prof Gavin Perkins
PARAMEDIC is evaluating the LUCAS mechanical chest compression
device for use during resuscitation by paramedics after out-of-hospital
cardiac arrest. The trial will recruit over 3600 cardiac arrest patients from
the West Midlands, Scotland and Wales.

High frequency ventilation
in ARDS
Dr Duncan Young & Prof Sallie Lamb
OSCAR will recruit 802 patients with
Acute Respiratory Distress Syndrome
by the end of August 2011 to identify
whether high frequency oscillatory
ventilation decreases mortality at 30
days compared with patients treated
with conventional artificial positive
pressure ventilation.
Beta Agonist Lung injury Trial (prevention)
Prof Gavin Perkins
Patients undergoing an elective oesophagectomy are susceptible to
developing acute lung injury (ALI). Balti-Prevention is recruiting 360
patients to investigate whether inhaled salmeterol prior to an elective
oesophagectomy will reduce the incidence of early acute lung injury.
