Comment: This test should be measured to establish the pathogenesis of fasting hypoglycemia. The
NACB guidelines suggest that there is no role for routine insulin in most diabetic patients. Diabetes Care
34:e61-e99,2011. Patients receiving insulin therapy may develop anti-insulin antibodies which could
interfere with the assay. This assay shows nearly equimolar cross reactivity (75-150%) with recombinant
human insulin (Actrapid and humulin) and insulin analogs (detemir, aspart, glargine and lispro).
