COVER SHEET FOR REQUESTS FOR LETTERS OF REFERENCE (FOR UNDERGRADS)
We are happy to provide you with letters of reference for your job/internship/grad school applications. For
each letter that you request, the department has a general set of guidelines (note that individual faculty may
have their own guidelines for you to follow as well, it’s worth double checking).
Your requests should be submitted at least 3 weeks before the letter is due. If you ask well in advance
(which is advisable!) SEND A REMINDER 2 weeks before the deadline.
For each request for a letter of reference, please fill out this sheet, and check off all of the boxes below.

FOR HARD COPIES:
□ If the professor is sending the letter, attach an envelope with their name and address typed on it as
addressor and the contact’s name, institution, and address as addressee.
□ If you are sending the letter, attach an envelope with the professor’s name and address typed on it as
addressor and the contact’s name, institution, and address as addressee. (If you need the professor to mail
you the letter, also include an envelope with their name and address typed on it as addressor and your
name, institution, and address as addressee.)
□ If a form is provided, attach it and type in your name, address and date, AND the professor’s name, title,
and address as listed on the AU website.
□ Attach a copy of your proposal/cover letter (regardless of whether it resembles prior ones).
□ If the professor has not read your work recently, attach a recent writing sample or one from a class that
you took with the professor.
□ If there is a waiver of confidentiality rights, please sign it. (Since non-confidential letters are given little
weight, it is a dubious use of faculty time to write them.)
□ If you are requesting letters for admission to more than 2-3 doctoral programs, please send a Word file
with a list of the addresses that the professor needs to type into the letters.
