UNCW FSL Intake Checklist
1. Be in good standing with the University
2. Have an updated Chapter Advisor on file with the University
3. Submit formal interest meeting information five (5) days prior to event.
4. Submit Candidate/ Aspirant Compliance form within two (2) business days
after formal interest session/rush event (yellow form)
5. Submit Fraternity and Sorority Hazing Compliance Form within two (2) days
after Formal Interest Meeting (blue form)

This form must be signed by all attendees at your interest session/rush event.
Hazing in any form is strictly prohibited. Hazing is a broad term that encompasses a multitude of
actions or activities. The term hazing refers to any actions or activities that may negatively affect
the development of a person or an organization; which cause mental or physical harm; or which
subject individuals to harassment, embarrassment, ridicule, or distress. Examples of situations that
are considered hazing include, but are not limited to tests of endurance, physical abuse,
psychological abuse, morally degrading or humiliating activities, forced ingestion of any
substance, activities which interfere with academic pursuits, and servitude.
Hazing is typically associated with membership selection and initiation into an organization. It is
possible for hazing to occur before, during, and after membership selection and initiation.
UNCW will investigate all hazing allegations. Individuals and organizations found in
violation of the UNCW Hazing Policy will be sanctioned and turned over to the proper
law enforcement agency. If you believe you or someone you know has been a victim of hazing,
contact the Coordinator for Fraternity/Sorority Life at (910)962-7295.

FRATERNITY AND SORORITY HAZING COMPLIANCE FORM
We certify that all activities sponsored or required by our inter/national fraternity/sorority and/or
members or pledge/associate members comply with the UNCW Hazing Policy, and with the
State law.
We have informed the candidate/aspirant member(s) of our fraternity/sorority of the contents of
the UNCW Hazing Policy. This policy will be read to aspirants at the beginning of each
semester’s membership intake process.
We understand that the chapter is not to engage in any pre and/or post pledging activities.
We understand that failure to uphold the UNCW Hazing Policy as stated will result in
referral to the Dean of Students for an organizational violation of the UNCW Hazing
Policy (i.e. the fraternity/sorority will face charges), and/or referral to the Dean of Students for an
individual violation of the UNCW Hazing Policy (i.e. the individuals within the fraternity/sorority will
face charges). We understand that our inter/national organization will be immediately notified if
there are any concerns or allegations of illegal membership activities occurring.
We understand that participation in any hazing activity or having knowledge of any hazing
activity and taking no action to stop the hazing is in effect giving our approval to haze. We
understand our responsibility to not allow members of our organization, whether graduate status
or affiliated at another institution of higher education, to haze our aspirants. Failure to report any
such activity of which you become aware may cause personal referral to the Dean of Students.

UNCW Hazing Policy
Engages in hazing. Hazing by university groups is prohibited on or off campus (see State Law
IV). Hazing is defined as a covert or overt action, occurring on or off campus, by an individual
or group of individuals in connection to recruitment, initiation, rite of passage, or membership in
a fraternity, sorority, UNCW sport club, group, organization or athletic team; that subjects any
other member of the university community, voluntarily or involuntarily, to activity which creates
an atmosphere for potential or actual humiliation, degradation, verbal, emotional or physical
distress, abuse or injury; or compromises the academic mission and/or reputation of the

NORTH CAROLINA GENERAL STATUTE 14-35.
HAZING; DEFINITION AND PUNISHMENT
It is unlawful for any student in attendance at any university, college, or school in this State to
engage in hazing, or to aid or abet any other student in the commission of this offense. For the
purposes of this section hazing is defined as follows: “to subject another student to physical
injury as part of an initiations, or as a prerequisite to membership, into any organized school
group, including any society, athletic team, fraternity or sorority, or other similar group.” Any
violation of this section shall constitute a Class 2 misdemeanor. (Effective December 1, 2003).
NORTH CAROLINA GENERAL STATUTE 14-38
WITNESSES IN HAZING TRIALS; NO INDICTMENT TO BE FOUNDED ON SELFCRIMINATING TESTIMONY
In all trials for the offense of hazing any student or other person subpoenaed as a witness in
behalf of the State shall be required to testify if called upon to do so: Provided, however, that no
student or other person so testifying shall be amenable or subject to indictment on account of, or
by reason of, such testimony. (1913, c. 169, s. 8; C.S. s. 4220.)
