Request for Qualifications
Statements of Qualification for DESIGN AND CONTRACT ADMINISTRATION OF THE CAMPUS WIDE
EXTERIOR SECURITY CAMERA PLAN for the Facilities Department will be received by the College of DuPage,
District 502, at the office of the Purchasing Manager, Berg Instructional Center (BIC) Building, Room 1540, 425
Fawell Blvd., Glen Ellyn, IL 60137, until 2:00 PM, Wednesday, April 22, 2015, at which time they will be reviewed
for consideration. In the event of office closure due to inclement weather, the deadline will be extended to the next
business day at the same time.
Any Qualifications received after the date and time stated above will be returned unopened. College of DuPage
shall not be responsible for responses that are not received at the specific office location indicated above by the
stated deadline. It is solely, the Respondent’s responsibility, to ensure that adequate time is allowed for timely and
accurate delivery.
No cost proposals are to be submitted with the qualifications. Cost proposals will be requested only after a
consultant selection is made
REQUESTS FOR INFORMATION/CLARIFICATION
If any Respondent submitting a statement of qualifications for this project is in doubt as to the true meaning of the
specifications or other documents or any part thereof, Respondent shall request clarification from the Buyer, Kelly
Froehlich. Questions must be submitted in writing and be directed via email to the Buyer at: Purchasing@cod.edu
no later than Thursday, March 8, 2015 at 5:00pm. Questions for which answers are provided will be communicated
to interested vendors via addendum.

SUBMISSION OF PROPOSALS
Respondents shall submit:
TWO (2) copies of this entire RFQ, including the Statement of Qualifications, and Certifications pages
are to be submitted, in an envelope, to Bid Proposal Box located at: College of DuPage, Room BIC
1540, 425 Fawell Blvd., Glen Ellyn, Illinois 60137, attention Purchasing Manager, no later than the
date and hour for the shown in these documents. The envelope submitted by the firm shall carry the

following information on the face of the envelope: Firm's name, address, RFQ number and name,
advertised date of RFQ Closing and the hour designated for RFQ Closing as shown in the legal
advertisement. When hand delivering proposals, Vendors will need to allow sufficient time to locate available
parking and report to the stated location. The college will not be responsible nor make allowances for any delays
in delivery of proposals
In lieu of the above, you may also submit an electronic proposal by emailing the Buyer, Kelly Froehlich at
purchasing@cod.edu no later than 2:00p.m. April 22, 2015. Please include the RFQ number and title in the
subject line of the electronic submission.
INSURANCE
GC-15 INSURANCE REQUIREMENTS

Prior to the commencement of work the Successful Offeror will provide, at its sole cost and expense,
Certificates of Insurance which shall remain in force throughout the term of the agreement, or any extension
thereof. The Contractor shall require all policies of insurance that are in any way related to the work and are
secured and maintained by Contractor and all tiers of subcontractors to include clauses providing that each
underwriter shall waive all of its rights of recovery, under subrogation or otherwise, against College of DuPage,
Board of Trustees and employees of the College.
The Contractor shall waive all rights of recovery against College of DuPage, Board of Trustees, employees of
the College and other Contractors and Subcontractors which Contractor may have or acquired because of
deductible clauses in or inadequacy of limits of any policies of insurance that are in any way related to the work
and that are secured and maintained by Contractor.
The Contractor shall require all tiers of Subcontractors to waive the rights of recovery against College of
DuPage and all tiers of Subcontractors.
Insurance Requirements of the Contractor
Prior to the effective date of this Contract, the Contractor, at its cost, shall secure and maintain at all times, unless
specified otherwise, until completion of the term of this Contract the insurance specified below.
Nothing contained in these insurance requirements is to be construed as limiting the extent of the Contractor's
responsibility for payment of damages resulting from its operations under this Contract. The insurance purchased
and maintained by the Contractor shall be primary and not excess or pro rata to any other insurance issued to the
College.
The Contractor's insurance shall apply separately to each insured against whom claim is made or suit is brought,
except with respect to the limits of the insurer's liability.
The limits of liability shall be as stated below, unless, prior to the effective date of this Contract, written approval is
granted by the College of DuPage Department of Risk Management for variance from those limits.

Coverages
(a)
Workers Compensation Insurance
Workers' Compensation shall be in accordance with the laws of the State of Illinois or any other
applicable jurisdiction.
The Workers Compensation policy shall also include the following provisions:

Employers' Liability coverage with a limit of
$500,000 each Accident
$500,000 each Employee
$500,000 Policy Limit for Disease

Broad form all states coverage
(b)
Commercial General Liability Insurance
(1)
The Commercial General Liability shall be on an occurrence form basis to cover bodily
injury and property damage including loss of use.
General Liability limits shall not be less than $1,000,000 per occurrence and
$2,000,000 aggregate combined single limit for bodily injury and property
damage. The General Liability policy shall include, without limitation the
following coverages:

Broad Form Blanket Contractual Liability;

Products/Completed Operations;

Broad Form Property Damage Liability;
Cross Liability.
Comprehensive Automobile Liability Insurance
Comprehensive Automobile Liability to cover all owned, non-owned and hired automobiles, trucks
and trailers. The Comprehensive Automobile Liability limits shall not be less than the following:

In addition to the coverages and limits specified above, Contractor and Sub-Contractors of any
tier shall secure and maintain a limit of liability no less than:

Additional requirements
(a)
Additional Insured
College of DuPage, its officials, employees and agents shall be named as additional insured
under the Commercial General Liability policy.
(b)
Qualification of Insurers
All insurance companies providing coverage shall be licensed or approved by the Department of
Insurance, State of Illinois, and general rating of "A", and shall be a Class V or higher in the
financial size category as listed in A.M. Best's Key Rating Guide, current edition or interim report.
Companies with ratings lower than “A”, V will be acceptable only upon written consent of the
College of DuPage Department of Risk Management.
(c)
Insurance Notices
All policies of insurance which may be required under terms of this Contract shall be endorsed to
provide that the insurance company shall notify the Purchasing Manager, College of DuPage
Room BIC 1540, 425 Fawell Blvd., Glen Ellyn, Illinois 60137 at least 30 days prior to the effective
date of any cancellation or modification of such policies. When required in the Special
Conditions, the successful Bidder shall furnish prior to the date on which Contractor commences
performance of the contract, Certificates of Insurance maintained by Contractor.
In no event shall any failure of the College to receive Certificates of Insurance required hereof or
to demand receipt of such Certificates of Insurance be construed as a waiver of Contractor's
obligations to obtain insurance pursuant to these insurance requirements.

OBJECTIVE
The College of DuPage is accepting statements of qualifications from firms to provide services to evaluate and
design a exterior security camera system utilizing existing networks and infrastructure. This RFQ is a solicitation for
qualifications and is not a contract or an offer for a contract.
Any agreement resulting from this RFQ will be at the discretion of the College of DuPage. The ultimate goal of a
selected respondent is to furnish a design package for solicitation of an exterior security camera system on the
Glen Ellyn campus.
EVALUATION OF RESPONSES
This Request for Qualifications shall be used in the selection of a designer for an exterior security camera system.
The evaluation of Responses will be based on the total qualifications submittal of the respondent’s design firm. All
properly submitted qualification packages will be reviewed, evaluated, and ranked by a committee of College of
DuPage representatives. College of DuPage may, at its option, request additional information or an interview with
one (1) or more of the top respondents prior to final selection.
Qualifications shall not include any information related to the Respondent’s fees, pricing, or compensation.
College of DuPage reserves the right to reject any and all responses and re-solicit for new Qualifications. College of
DuPage makes no representations, written or oral, that it will enter into any form of an agreement with any
Respondent of this RFQ.
By submitting its Qualifications in response to this RFQ, Respondent accepts the evaluation process and
acknowledges and accepts that determination of the “most qualified” respondent will require subjective judgments
by College of DuPage.

GENERAL INSTRUCTIONS
1.
Submittals shall be prepared simply and economically, providing a straightforward description of the
Respondent’s Qualifications as related to the questions and ability to meet the requirements of this RFQ.
2.
Emphasis shall be based on quality, completeness, clarity of content, responsiveness to the questions and
requirements, and an understanding of project needs for College of DuPage.
3.
Respondents are requested to submit (4) copies of qualifications to be received by College of DuPage
Purchasing Department on specified date and time.
4.
Qualification and any other information submitted by respondents in response to this RFQ shall become
property of the Owner.

Failure to comply with all requirements contained in this Request for Qualifications may result in rejection
of Qualifications submittal

Under no circumstances are respondents to contact or discuss this RFQ with any College of DuPage employee,
other than using the methods outlined in this RFQ, any of the information contained herein or about this project in
general. Respondents are strictly forbidden from visiting the College’s locations or approaching any College
employee, vendor, contractor or subcontractor for any information related to this RFQ or this project without the
direct knowledge and authorization in writing in advance from the Purchasing Manager or Buyer. Violation of
these provisions may subject the respondent to immediate disqualification.

Vendors will not use the College of DuPage name or logo in any form or any manner whatsoever without prior

This addendum is issued to address questions.
1. Do you know how many cameras are estimated to be installed?
Approximately 30.
2. What video management software is used currently?

Please submit 2 (two) printed copies or one email Bid by the due date.
4. What is the actual due date for questions?
Questions were due April 15, by 5:00pm. However due to a typo in the documents, questions will be
accepted until 4/17/2015 at 3:00PM CST.
5. We would not typically hire the electrical engineer, civil engineer, etc. These other
disciplines are sometime brought on by the Owner. Is that an option for this project or is it
required that we list other designers / engineers as part of our team?
The College is requiring that the winning Firm bring on their own Engineers.
6. Can the College of DuPage tell us which designers/engineering firms have performed work
for the College in the past and which firms have performed satisfactorily?
It is the responsibility of the bidding Firm to make these decisions. Firm should not limit themselves
to only C.O.D. recommendations, and therefore will not be able to provide a list.
