Additional Salary Information: This is a grant funded program which expires at the end of 2012.
Description of Duties: Under the direction of the Health Officer and the Portage County CAN Coalition, this position serves
as the primary representative and point of contact, providing advanced support services to the Health Officer and
professional staff associated with implementation of the Fit Families- Fit Communities obesity prevention
grant. Responsibilities include complex organizational and administrative tasks for the health department and Portage
County CAN Coalition related to the implementation of the Fit Families- Fit Communities work plan and other programs
as directed by the Health Officer and the Portage County CAN Coalition.
Minimum Qualifications: Bachelor’s degree in public health, planning, nursing, environmental health, public
administration, community wellness or related degree Proven ability to organize/facilitate groups Proven ability to plan
and complete projects One year experience in public/community health field preferred Ability to manage complex projects
and maintain agreed upon timelines General knowledge of community resources and their roles and functions as related
to public health Must be a licensed driver and have access to an insured vehicle

POSITION SUMMARY (Describe major responsibilities of this position)
Under the direction of the Health Officer and the Portage County CAN Coalition, this position serves as the
primary representative and point of contact, providing advanced support services to the Health Officer and professional
staff associated with implementation of the Fit Families- Fit Communities obesity prevention grant.
Responsibilities include complex organizational and administrative tasks for the health department and Portage County
CAN Coalition related to the implementation of the Fit Families- Fit Communities work plan and other programs as directed
by the Health Officer and the Portage County CAN Coalition.
SUPERVISION RECEIVED:
Receives direct supervision from Health Officer.
SUPERVISION EXERCISED:
None over agency staff.
ESSENTIAL FUNCTIONS:
85% A. Implementation of the Fit Families- Fit Communities Grant
1. Facilitates implementation of work plans developed by Portage County CAN.
2. Participates in the evaluation of services and the assessment of the health of the community.
3. Collaborates with other community agencies and resources to implement work plans.
4. Applies public health principles to individuals, families and groups in the community setting.
5. Collects, analyzes, and reports program-specific data to assure program outcome goals are met or to identify
program management problems that need to be addressed.
6. Arranges and participates in multi-disciplinary meetings as indicated.
7. Makes presentations to interested community groups on topics relevant to obesity prevention.
8. Completes record keeping for individuals focus programs that assures documentation and tracking of required information.
10% D. Fit Families- Fit Communities Grant Coordination
1. Serves as a resource for staff, community, and other community providers.
2. Collaborates with supervisor in determining program needs, i.e., supplies, personnel, etc.
3. Schedules and chairs program committees, sets agenda, and arranges for recording of meeting minutes.
4. Prepares required reports for contract obligations.
1% F. General Duties
1. Attends staff meetings.
2. Completes work log, payroll, time cost studies, and other records required for sectional and departmental function.
3. Maintains client confidentiality.
4. Complies with all agency policies.
NON-ESSENTIAL (PERIPHERAL) FUNCTIONS:
4% A. Preparation and Maintenance of Records, Reports, and Statistics
1. Collects, compiles, and maintains statistics and reports as required and/or requested by state and/or federal agencies
and supplemental programs.
2. Develops and evaluates new and existing records, reports, and data systems on local, state, and national levels.
4. Assists in the development and writing of proposals for grants and contracts.
MINIMUM QUALIFICATIONS
Education/Experience/Certification:
(A) Graduate from accredited college or university with a Bachelor’s degree in public health, planning, nursing,
environmental health, public administration, community wellness, or related degree.
(B) Proven ability to organize/facilitate groups.
(C) Proven ability to plan and complete projects.
(D) One year experience in public/community health field preferred.
Knowledge, Skills, and Abilities:
Ability to work independently and efficiently with a broad variety of community members.
Ability to manage complex projects and maintain agreed-upon timelines.
Ability to collect and analyze data.
Basic knowledge of organizational theory and development.
General knowledge of community resources and their roles and functions as related to public health.
Ability to communicate effectively in verbal and written form.
General knowledge of Portage County area.
Ability to coordinate and organize public health programs.
Availability to work weekends and hours outside of regular agency working hours as necessary.
SPECIAL REQUIREMENTS
This position is in the No Exposure category for bloodborne pathogens and will follow all recommended training
and procedures for this category.
Must be a licensed driver and have access to an insured vehicle.
