Field Evaluation
(To be completed by Field Supervisor)
All social work students must complete at least 400 hours of field experience. To best evaluate and
measure student progress over time, this evaluation should be completed twice during the placement
(once at about the halfway point and once upon completion of total hours). Getting feedback from you is
an essential part of the student’s learning experience. This evaluation is also an important part of the
UWSP Social Work Program’s assessment process; we utilize results to identify areas in need of
improvement in the curriculum.

Instructions
The standard by which your student is to be compared is that of a new entry-level social worker. Please
utilize your student’s learning plan as a guide as you complete this evaluation. The 10 competencies and
41 practice behaviors used in this evaluation are those established by our national accrediting organization,
the Council on Social Work Education. Please rate your student on each of the 41 practice behaviors,
using the following scale.
