Condition Care Rx Program
The Condition Care Rx Program was designed to help you manage the cost of medications used to treat
certain conditions. This may help keep your health on track and encourage medication adherence.
The Condition Care Rx program gives you the ability to obtain your medications at a reduced cost share.
These cost shares are reduced by waiving the deductible on HSA plans or reducing the copays on copay plans.
The medication lists for HSA and non-HSA plans are not the same as the treatment categories differ:
•The Condition Care Rx program for non-HSA plans includes medications for diabetes, cholesterol, respiratory,
high blood pressure, and depression.
•The Condition Care Rx program for HSA plans includes medications for diabetes, cholesterol, respiratory, and
high blood pressure.

• A monthly member health statement that shows what
was paid, what is owed and ways to save.
• A member website for 24-hour access to benefits,
claims, ID cards, health information, real-time cost
comparisons, wellness programs and more.
• E-visits that allow members to schedule appointments,
request prescription renewals and refills, engage in a
“virtual” office visit with the doctor, receive lab test
results and more.
