7134.1 - Qualifications

Incidental operators and motor vehicle operators of standard 4 x 2 vehicles up to and including

12,000 GVW shall be qualified, tested, and certified through the state licensing program. Proper documentation showing each operator's qualifications shall be kept in their official personnel folder.

7134.2 - Training

All operators of fleet equipment will be provided with training that fully informs them of their responsibilities. The instruction shall include procedures for obtaining repairs, correcting deficiencies, driving defensively, driving to conserve energy, making preventative maintenance checks, and safe operation of fleet equipment in a Forest environment. Provision of training is the responsibility of the unit managers.

7134.3 - Policy

1. Identification cards SF-46 or OF-346 will be issued with an indefinite expiration date only to special equipment operators. A special equipment operator means an employee who operates the following types and/or sizes of motorized equipment:

Buses, Emergency vehicles, Law Enforcement vehicles, Vehicles used for transportation of dangerous material, Highway vehicles greater than 12,500 GVW, 4 x 4 vehicles of any type or size, Trailer towing, Horse hauling, Construction, industrial and agriculture equipment (e.g. graders, crawler tractors, wheeled tractors, wheeled loaders, tracked loaders, log skidders, forklifts), All other fleet equipment requiring an operator (e.g. snowmobiles, motorcycles, ATVs, motorboats)

2. Form AD-184 (Application for Motor Vehicle Operators I.D. Card) will be required for all special equipment operators. Seasonal and temporary employees shall update yearly.

3. State record checks will no longer be required.

4. Medical examinations for heavy equipment operators shall be required upon initial employment and every 2 years thereafter.

5. Operators shall have had a defensive driving course before they are issued authority to operate Government owned or leased equipment. A refresher course shall be required every 3 years.

6. Work supervisors should evaluate the operating performance of each new operator within the first 30 days following authorization to operate.

7. Examiners shall be designated by the Forest Supervisor and/or the unit managers.

Designation shall be in the form of a memorandum, and will state what the individual is designated examiner for.

8. Written tests are no longer required unless specified by FPM 930, AGPMR, or FSM 7134.

9. Volunteers shall be licensed the same as regular employees, POVs included.

10. Follow the general direction of 7109.18 Driver-Operator Handbook, and 6709.11 Health and

Safety Code Handbook.
