05 - Definitions.
Claims Information System Data Base. A claims information system, using data
processing, will produce reports on open and closed claims in different formats for
management purposes. A unit report shall be sent periodically to each Forest
Administrative Officer (at least once each quarter) through the fiscal year. The unit
report summarizes all open and closed claims that have occurred for that unit
within the current fiscal year. This report should be shared with the Forest
Supervisor, Budget and Finance Section Head, and the employee responsible for
processing claims.
A brief explanation of the unit report format is as follows:
Column 1 - Control No.: Identifies the Control Number assigned to each
claim received in the Regional Office. The first two digits represent the fiscal year
the Regional Office received the claim, followed by a 4-digit number which
represents its chronological identity during the fiscal year. For example, Control
No. 910001 represents the first claim received in Fiscal Year 91.
Column 2 -CO: Identifies the Claims Officer assigned to the case.
Currently, the Claims Officers are identified as follows:
1 = To be assigned
2 = To be assigned
3 = Garth Lascink
4 = Jim Miller
5 = To be assigned
6 = Mary Lee Maloney
Column 3 -Claimant Name: Identifies the claimant's name for claims
against the Government and the debtor's name for claims by the Government.
Column 4 -Date Happen: Identifies the date of occurrence leading to the
claim.
Column 5 -Date Received by RO: Identifies the date the claim was received
in the Regional Office.
Column 6 -Resolved by Justice: Identifies the date on which the claim was
adjudicated by the Department of Justice.

Column 7 -Date Last Action: Identifies the date of the latest significant
action involving the subject claim.
Column 8 -Date Closed: Identifies the date the Regional Office closed their
file on the subject claim.
Column 9 -F/A: Identifies whether the claim is for "F" or against "A" the
Government.
Column 10 -Type: Identifies the type of claim involved. Following is an
explanation of the coding:
Code
Description

TM--All timber trespass, timber sale contract, and other timber
program related contract claims, including defaults on thinning
contracts, and so forth.

Fire--Claims for suppression costs, resource damage caused by
fire, escaped slash fires, claims on defaulted contracts related to
fire.
3
MVA--Claims that result from a collision between a Government
vehicle and another vehicle. This code will also be used if there
is a personal injury and/or wrongful death claim associated with
the accident.

Employee--All claims filed by employees, including damages to
their motor vehicle.

PP--All claims for damage to personal property not related to
any of the above categories. Occasionally, property damage
claims also include claims for personal injury and/or wrongful
death. If the amount claimed for injury/death exceeded the
amount claimed for the property damage, then either code 6 or 7
is used, as appropriate.

PI--All claims for personal injury not related to a motor vehicle
accident.

Fatality--All claims for wrongful death not related to a motor
vehicle accident.
8
Other--All other claims that do not fit in categories 1 through 7
above. Primarily, this will be for claims referred to the Regional
Office by contracting officers on contracts not related to timber
or fire.

Potential--A case that has the potential for becoming a claim.
Column 11 -Amount Claimed: Identifies the "sum certain" amount claimed
or the amount billed the debtor.
Column 12 - Amount Settled: Identifies the amount paid the claimant or the
amount collected from the debtor.
The unit report is arranged with all claims against the Government shown grouped
together at the beginning of the report and all claims for the Government grouped
last. For each grouping, there is a subtotal for each type of claim and a total for
each group.
Each Forest shall maintain a record of those claims that are within the Forest
Claims Officer's delegated authority.
