Department of Patient Counseling Student Application Checklist
(All applicants must complete & submit all related processes below before interviews are scheduled)
Please keep a copy of all materials submitted for your records in case of lost or
misplaced documents.
______Apply to University/Graduate School Online Application. Pay $65.00 Graduate School application
fee online (keep proof of payment), or by check (address is listed below for VCU Graduate School
Office of Admissions).
______Order Transcripts/All transcripts must be official, sealed in envelope and mailed to Graduate
School Admissions.
The following items must be submitted to the Department of Patient Counseling:
______Complete ACPE face sheets/both sheets must be signed and dated.
______Complete & submit essay/A reasonably full account of your life.
______Complete & submit essay /A description of your spiritual growth and development.
______Update and submit/A description of your work (vocational history) and a resume.
______Complete and submit/An account of a “helping incident” (certificate students).
______Complete and submit essay/Your impressions of CPE (Clinical Pastoral Education).
______List three references on the ACPE application in the Academic Reference section (1 academic, 1
denominational, and 1 personal). This is a department requirement separate from the 3 written
academic reference letters required by the Graduate School. The academic reference listed on
the ACPE application can be solicited as one of the written references required by the Graduate
School.
______Solicit three academic references using the VCU Graduate School reference form. References
should be mailed or emailed to Department of Patient Counseling.
______Submit two verbatims (MS degree/residency applicants).
______Submit one self-evaluation and one supervisor’s evaluation (MS degree & residency applicants).
______Submit GRE score or proof of at least one year of graduate education with a B or better gradepoint average.
*______International Students/Obtain all documentation (Visa, SSN etc.) allowing you to attend/and or
work at the University/Hospital.
_______International Students/arrange to take the TOFEL if you did not receive your degree at a U.S.
university.
_______International Students/need external credential evaluation. Use WES, ECE or AACRO. Send your
evaluations to the Office of Admissions, address is listed below.
*______International Students/Verify that your evaluation is complete based on your application
materials, international graduate school requirements and your citizenship status.
*International Students need to contact the International Graduate School office to receive additional
information needed to complete the application process as it applies to international students.
*International Students/correspond with I Wayan Mastriyana (Mas) in International Graduate School
regarding status of your evaluation process. You will not be admitted to the program until your
evaluation is completed.
