shared with Milton Keynes Council to enable it to fulfil its statutory obligations to secure the provision of
information, advice and training for providers of childcare and to review the sufficiency of childcare and nursery
education provision in their area.
Under the Childcare Act 2006, Section 12, every Local Authority has a statutory obligation to provide information
to the public regarding the provision of childcare and related services in the area. In fulfilment of this duty,
should you give your consent to C&FI, the information you provide on this form will be made available to the
public over the telephone, in writing and via the internet.
Please note that unless this form is completed in full, signed and returned to C&FI, it will not be possible to
make your information available to enquirers looking for childcare and other family-related services.
